One of the best things about living and working in this village is its communitarian vibe. Just seeing these little slate signs about the nature all around us really brings a smile to my morning dog walk.

Clip from this mornings work based gym session. 20kg kettlebell Halo to hip in kneeling. It’s tough!

28/01/2026

I’ve had car troubles so getting to the gym has been impossible this last couple of weeks, so training at work has been the only way to get a session in.

Pavel Tsatsiuline’s Simple & Sinister has provided the foundation of the regime, with daily one handed kettlebell swings (100 reps in 5 min) and Turkish Get Ups (10 reps total, no time limit). It’s a great program which is quick to compete and has incredible carry over to multiple activities. Completed today with 4 x 8 reps of a barbell Zercher Squat, which has rapidly become one of my favourite / most hated movements!

Degenerative meniscal tears are very common on MRI in middle-aged and older adults, yet most are not painful, highlighting that wear or damage alone does not explain symptoms. A large community study found that 61% of people with meniscal tears reported no knee pain, and newer research shows these tears are usually complex and multifactorial rather than a single “problem lesion.”

A recent narrative review looking at current evidence concludes that when pain does occur, it arises from interacting mechanisms—including altered joint loading, cartilage and bone stress, synovial inflammation, abnormal nerve ingrowth, and changes in central pain processing—rather than from the tear itself or simple “mechanical symptoms.” This helps us to explain why imaging findings often correlate poorly with pain and why management needs to be individualised and mechanism-informed, not imaging-driven.

“Pain in degenerative meniscal tears cannot be attributed to a single structure, but rather reflects the interaction of biomechanical and neurobiological processes.” ~ Tsai et al. (2025)

4 Years since this photo was taken at our very first TEDxHayfield event at the Royal Hotel, Hayfield. We had 4 fantastic speakers that evening, covering topics such educational leadership, the psychology of how to shape your own environment, whether planting millions of trees is the solution to the climate crisis and how to get better sleep. Since this talk one of our speakers (Holly Hartley) sadly lost her battle with cancer.
You can still view the inspiring talks on our website at:

In clinical practice, we are beginning to see musculoskeletal problems related to the use of weight loss drugs, including muscle loss and decreasing balance. Some patients are reporting reporting feeling weaker and more unsteady than before.

This may be an early glimpse of a wider trend. Rapid fat loss can accelerate muscle and bone loss, leading to sarcopenia (loss of muscle mass) and osteopenia/osteoporosis (loss of bone density).

When these occur together, we call it osteosarcopenia — a condition that significantly raises the risk of falls, fractures, and frailty, particularly in postmenopausal women. Hip fractures already carry a 30% one-year mortality rate and cost the NHS millions annually.

Even more concerning, if weight is regained after stopping medication, the returning mass is often fat, not muscle, leaving patients with sarcopenic obesity — weaker, frailer, and metabolically worse off than before.

Resistance exercise is not optional — it’s essential.

Regular muscle-loading activity should be a cornerstone of all weight-loss and maintenance plans to preserve strength, balance, and bone density.
