🎉 Working in a care home? These new FREE tools are just for you! 🎉

To celebrate Care Home Open Week (16–22 June), we’ve launched three brilliant new resources to help you plan care that really puts people first.

They’ve been created with help from over 100 care staff and families – so they’re practical, easy to use, and made with real care in mind:

🧡 A quick guide to care planning
🧡 A step-by-step guide with real examples
🧡 A colourful poster with top tips for staff, residents and families

Have you or a relative been discharged from hospital recently via a "discharge to assess" scheme (where follow on care is planned after you leave hospital and where you get up to 6 weeks NHS funded care)?

As part of an NHS funded project, we are keen to speak to people to learn about your experiences.

Please drop Jenny an email if you are able to help with our study!
