🔴What I Won’t Do as a Post-Op Therapist!🔴

When you choose a post-op therapist, you aren’t just looking for someone to help with recovery—you’re looking for someone who respects your body’s delicate healing process.

To keep my practice safe, professional, and effective, there are certain things I simply will not do:

❌Push fluid out of open wounds: Your incisions are for healing, not for forced drainage. I work with your body’s natural internal pathways. If a tiny bit of fluid seeps out during the session, that is fine, but I am not actively looking to push large amounts of fluid out!

‼️Reopen incisions: I am a therapist, not a surgeon. I respect the integrity of your surgical sites.

❌Proceed if there are signs of infection: Your health comes first. If I see a red flag, I will refer you back to your medical team immediately. The session is paused until confirmation is given by your surgeon or consultant.

‼️Perform hard or painful massages: True Manual Lymphatic Drainage (MLD), especially post-op, is gentle. Pain is a sign of tissue stress. We are here to reduce stress, not add to it.

❌Use “Wood Therapy” on fresh post-op tissue: Your delicate tissues need a soft touch, not aggressive tools.

‼️Brazilian style techniques: Post-op care requires a specialized approach tailored to your surgery. Brazilian Sculpting Massage is an absolute NO!

❌Claim to have all the answers: Every body heals differently. I work with you (and your surgeon’s guidelines) to find the best path forward. If I don’t know the answer, I will find out for you, pause treatment, or refer you to someone else.

‼️Simply ‘wing it’ if I’m in doubt: If I suspect you need to speak to a doctor, I’m not going to proceed with treatment. Yes, I’m neurotic and perhaps a bit too careful, but I’d much rather send you home only to find out it was nothing, than perform the treatment and have you end up in the hospital!

I WAS IN TRAINING THE OTHER NIGHT when we started talking about pre-op Manual Lymphatic Drainage (MLD).
The instructor shared something a plastic surgeon told her that I just had to share with you:

He said that performing surgery on someone who has had pre-op MLD is like clearing the leaves from the gutter before a storm.

Think about it: Your lymphatic system is your body’s drainage pipes.
When we ‘clear the gutters’ before your procedure, we are literally prepping the surgical site—removing excess fluid and metabolic waste so the surgeon has a clean, clear field to work in.

It doesn’t just make the surgeon’s life easier; it sets your body up for a smoother recovery with less initial swelling.

In traditional Manual Lymphatic Drainage (MLD), we use little to no oil, balms, or creams.
While it might feel different from a standard massage, this “grip” is fundamental to how your lymphatic system actually functions.

Here is why that skin stretch is so important:

🪢 Mechanical Traction: To reach the lymphatic vessels, I need a firm “grip” on the skin’s surface. Oil causes hands to slide over the skin rather than moving with it, which is what creates the essential pumping action your lymph fluid needs to move.

🚪The “Trapdoor” Effect: MLD uses a specific stretch to pull on tiny microscopic “anchor wires” beneath your skin. This tension physically swings open the “trapdoors” of your lymphatic system, allowing trapped fluid and toxins to finally drain away.

🏹 The Vacuum Effect: By stretching the skin and then releasing it, we create a pressure change. The stretch moves fluid forward, while the release acts like a vacuum, allowing the vessels to refill from the surrounding tissue.

Directional Mapping: Your skin must be stretched toward specific lymph nodes to drain properly. Without that elastic stretch, we aren’t moving fluid into the vessels—we’re just pushing it around under the skin.

🧹This was followed by some Dry brushing with a special brush designed for facial brushing

🧴Then finished off with some Gua Sha, and a moisturiser

THE MISSING LINK IN DEEP TISSUE MASSAGE:

For some people out there, myself included, a deep tissue massage is simply not enough.

You’re getting massage after massage, but you find that it’s just not releasing the tension. and stretching actually aggravates it more.

You start to wonder what’s going on...

With some people, the body actually just tenses up more when deep pressure is applied to areas of tension. This is especially true for people who are very stressed out and have higher levels of cortisol in their body.

The body reacts and tenses up more because it goes into protection mode.

In this case, breaking through that barricade known as the fascia is key. Think of fascia as the cling film that encases every organ, bone and muscle etc within your body.

By using slow compression, skin rolling, and negative pressure, we ‘unstick’ the superficial and deep connective tissue (fascia).

This melts away the surface resistance and “opens the door” for deeper work, allowing for a more profound release of tension without the body fighting back.

WHEN IT COMES TO SURGERY, the actual operation is the easy bit, it’s the recovery that presents with the most challenges. Here are some non negotiable’s when it comes to recovery:

🍷 No Alcohol: Alcohol thins the blood and increases swelling. Alcohol is absolutely off limits for at least 6 weeks.

❤️‍🩹 Compression Garments: These are non-negotiable. They reduce fluid buildup (seromas) and help the skin “shrink-wrap” to your new contours.

🏃The Rest/Movement Balance: Rest is for healing, but gentle movement prevents blood clots and stimulates the lymphatic system. But listen to your body =if it throbs, slow down.

🙌 Manual Lymphatic Drainage (MLD): Specialised post Op MLD is different to the standard classic MLD. These are the secret weapon for targeting swelling, bruising and fibrosis. It flushes out stagnant fluid and speeds up the healing process.

🏥 Follow Surgeon’s Advice: Every surgeon has a specific protocol for a reason. If they say “no showers for 48 hours,” there’s a clinical risk they are trying to avoid.

💦 Hydration & Protein: Healing requires massive amounts of energy. Focus on high-protein and anti inflammatory foods.

As I get older, I’m becoming increasingly aware of how my body is going to change as my hormones take control and unleash a whole host of challenges upon me.

At the same time, I have had a lot of clients come through my door, looking for ways to alleviate their menopausal symptoms, and I have found that Manual Lymphatic Drainage is simply one of the best ways to assist with these symptoms!

As a woman, and someone that is going to go through this ‘change’ myself very soon, I feel it is my moral duty to offer a treatment that can help all other women and people who are in perimenopause and suffering in silence.

MLD on its own offers great relief from menopausal symptoms, but I wanted to offer something even more, and create a treatment that really focused on menopausal symptoms.

That’s why I have been working on this new offering.
Custom building it, using my skills and qualifications in other therapies to try bring other women something that can really help support their journey through Menopause.

‼️The harsh reality of post op recovery!

I will say this until I am blue in the face!

❌ Post op recovery in no joke!
❌ It is not to be taken lightly!
❌ It is not linear!

Surgery is the easy part - the recovery is the hard part!

And as a post op therapist, client safety should be your first priority!

This morning, this client came to me for her first post op manual lymphatic drainage (MLD) after having her second lipodema surgery.
Her first surgery and recovery had gone off without any snags or complications.
But The moment I saw this, I stopped in my tracks.
Bruising and a small amount of redness is common after lipodema surgery, but I was concerned about the deep red patch on her left leg.
My first thought was cellulitis - (a potentially serious bacterial infection) but on closer inspection, the area was not hot to the touch - a common symptom of cellulitis.
I made the decision to halt the MLD session with immediate effect, and we contacted her wonderful surgeon in Cyprus who got back to her within a few minutes.
Although most probably not cellulitis, it is still looking very inflamed and irritated, and we are hoping that antibiotics won’t be needed, but I sent her home anyway, and she is waiting for further instructions from her surgeon.

This is the world of post op recovery and being a post op therapist.
Going with your gut, and putting the safety of your clients above anything else!

Even if it turns out to be nothing to worry about, I’d rather be safe than sorry.

This lovely client of mine has graciously agreed to be a case study as well (she had agreed to be a before and after anyway) but we will document her journey, and follow all surgeon advice and post operative protocol.
