Return to Running Post Baby Part 5

Grade & Monitor Your Return to Run

🏃‍♀️So you are symptom free, around 12 weeks post baby, built up your strength, passed the tests and now ready to hit the road running - literally!!!

🏃‍♀️So so exciting!! So where do you go from here?
Well we recommend a graded return to running, a walk run interval based program! We love the couch to 5k NHS app. I would advise a minimum x2 week alongside your strength work, max 3 times a week. Aim to space your runs out over the week. Throughout monitor how you feel, if you develop symptoms then please contact your women’s health physiotherapist for some fine tuning and advice.

🏃‍♀️Progressing from 5k onwards and speed work is likely to require further work with your women’s health physio and I would advice getting comfortable with 5k for a little while before pushing beyond this.

🏃‍♀️For more challenging performance goals eg completing a key race in a certain time it is so worth investing in a running coach. is incredible and offers some amazing tips on her account and if you are based near her in the U.S hook up with her. In the UK where I live the running coaches who I work with are smashing it out of the park, is awesome and are also fab.

🏃‍♀️Enjoy this process wonderful Mamas you are on the home straight, don’t rush it and when in doubt ask the professionals.

Does the research show that you can reduce the severity of a perineal tear?

💥90% of women will tear during their first va**nal delivery! The majority of tears are 1st degree ( small tears or grazes) that don’t require stitches and heal quickly or 2nd degree tears (which do require stitches) and take a little longer to heal.
💥Unfortunately 7% of women will experience a 3rd / 4th degree tears
(tear extends into the a**l sphincter). These can seriously affect the pelvic floor and postnatal recovery. There are however some things that you can do to reduce the severity of a perineal tear.........

👉🏻Perineal massage - A technique used to stretch the perineal area. It can be carried out from 35 weeks of pregnancy. Carried out 1-2 times a week it has been shown to reduce the likelihood of tearing (that requires stitches) or having an episiotomy by around 9% (Cochrane review 2013)

👉🏻Keep active during the first stage of labour and lying on your left side or upright during the pushing (2nd stage) have all been show to reduce the chances of prolonged pushing, assisted delivery and need for epidural all of which increase the risk of 3rd and 4th degree tears (Cochrane 2017)

👉🏻Warm compress placed on the perineum by a health care professional as baby’s head crowns reduces severity of tear (Cochrane 2017)

👉🏻Relaxation, acupuncture and breathing techniques, (similar to those used in hypnobirthing ) have been shown to reduce the chances of an assisted delivery, therefore 3rd/4th degree tear (Cochrane 2012)

👉🏻Have a plan, know what you ideally want but be prepared to keep the dialogue between your health care professional open.
💥We do know that epidurals and forceps delivery increase the risk of a 3rd or 4th degree tear BUT sometimes these may be necessary for the benefit and safety of you and baby. has a great blog on instrumental delivery which I would highly recommend reading!
