Delighted to be attending The British Pain Society Annual Scientific Meeting in Harrogate to talk about Inspira Health’s Primary Care Chronic Pain (PCCP) service which identifies patients with chronic severe neuropathic pain who are suitable for spinal cord stimulation (SCS) therapy. The service links specialists from secondary care to colleagues in primary care and has been shown to increase SCS utilisation, raise awareness in primary care and improve patient prognosis.

Between July 2024 and September 2025, our Cardio-Renal-Metabolic Quality Improvement Programme (CRM-QIP) completed 6,838 clinical reviews in primary care for patients living with chronic kidney disease (CKD) or markedly elevated albuminuria.

These reviews were delivered by NHS Consultant Nephrologists and Cardiologists from across England, with a focus on reducing cardiovascular risk and slowing the progression of kidney disease.

Overall, 61% of patients had changes made to their medication, including initiation or optimisation of ACEi/ARB therapy (n=1,600) and SGLT2 inhibitors (n=2,571). These treatment adjustments are estimated to reduce the five-year risk of kidney failure by at least 26%.

Patient Stories

Following our recent Heart Health Scan Clinics in GP practices across Wirral, we have seen many positive outcomes, with patients benefiting from hand-held echocardiograms and consultations with Consultant Cardiologists from the Liverpool Heart and Chest Hospital TAVI team. This patient story highlights the real-world value of proactively supporting our colleagues in primary care and the significant improvements that can be made to patients’ quality of life.

Promising results from our ‘hand-held’ echocardiogram clinics in the community on the Wirral.

A programme to proactively identify patients suitable for TAVI (Transcatheter Aortic Valve Implantation) or TEER (Transcatheter Edge-to-Edge Repair) offers significant benefits for patients, clinicians, and the wider health system:

1. Patient benefits
a) Improved survival: Earlier identification and treatment of severe aortic stenosis or mitral regurgitation reduces mortality risk.
b) Better quality of life: Relief of symptoms such as breathlessness, fatigue, and reduced exercise tolerance.
c) Less invasive treatment: These procedures avoid open-heart surgery, resulting in shorter recovery times and fewer complications, particularly in older or frail patients.
d) Reduced hospital admissions: Effective treatment lowers the risk of recurrent heart failure admissions.

2. Clinical benefits
a) Timely referral and intervention: Ensures patients are treated at the optimal stage of disease rather than when critically unwell.
b) Improved clinical outcomes: Planned procedures have better outcomes than urgent or late interventions.
c) Enhanced multidisciplinary care: Supports structured pathways involving cardiology, primary care, and heart valve teams.

3. Health system benefits
a) Reduced emergency care costs: Prevents costly unplanned admissions and prolonged hospital stays.
b) More efficient use of specialist services: Identifies appropriate patients earlier, improving pathway efficiency.
c) Supports population health management: Helps address underdiagnosis of valvular heart disease, particularly in ageing populations.
d) Alignment with best practice guidelines: Ensures equitable access to advanced, evidence-based therapies.

4. Strategic and organisational benefits
a) Improves case finding: Particularly in high-risk groups such as older adults and heart failure patients.
b) Reduces variation in care: Ensures consistent identification regardless of where patients enter the system.
c) Supports elective recovery and waiting list management: By identifying patients before they deteriorate.

Around one in four people in Knowsley who attend an NHS Health Check at their local GP practice are subsequently found to have an underlying cardiovascular condition.

Issues such as high blood pressure, raised cholesterol, and irregular heart rhythms like atrial fibrillation can often develop without noticeable symptoms, increasing the risk of heart attacks and strokes if left untreated.

Having an NHS Health Check is a simple but important step in spotting these conditions early. By being proactive and attending a check at your GP practice, you can take action sooner, reduce the risk of life-changing events, and make positive changes to support a healthier future.

The Primary Care Heart Failure (PCHF) service has carried out, over the last 8-year, more than 4,000 patient reviews by consultant cardiologists within local GP practices across England.

As a result, 650 patients have received complex cardiac device therapies, including Implantable Cardioverter Defibrillators (ICDs) and Cardiac Resynchronisation Therapy devices (CRT-P/CRT-D), which are used to treat severe heart failure and life-threatening heart rhythm disorders.

These devices help improve survival and quality of life by correcting problems with the heart’s pumping coordination (ventricular dyssynchrony) and by detecting and treating dangerous abnormal heart rhythms.

We are in Birkenhead Wirral today at Claughton Medical Centre and The Warrens Medical Centre carrying out heart health scans with Dr Kully Sandhu, Consultant Cardiologist from Liverpool Heart and Chest Hospital NHS Foundation Trust.

Heart valve disease is a growing concern in the UK, with an increasing number of people, especially those over 65, affected by it. It's estimated that around 1.5 million people aged 65 or over in the UK have some form of heart valve disease. The condition can lead to serious health issues if not diagnosed and treated early.

Checking for heart valve disease in primary care will identify health issues earlier and lead to improved long-term outcomes for patients.
