Primary Care Mental Health Triage Coach is a specialised role, often Band 4 within the NHS (e.g., in Rotherham Doncaster and South Humber NHSFT), that provides early, targeted support for mental health issues within GP surgeries. They offer individualised coaching, assess risk, and facilitate access to community resources to promote recovery and prevent crises.
Key Aspects of the Role
Purpose: To improve patient journeys by providing timely mental health and wellbeing support, acting as a bridge between primary care and specialised services.
Duties: These coaches work with individuals to set personalised goals, manage stress, build resilience, and develop long-term strategies for mental wellbeing.
Intervention: They deliver high-intensity, time-limited, and often community-based interventions, serving as an alternative to acute, crisis-driven care and aiding in early discharge or reablement.
Collaborative Practice: They work alongside GP practices, mental health professionals, and community partners to support patients with both physical and mental health conditions.
Expertise: Coaches need knowledge regarding factors impacting mental health, risk assessment, and care planning.
These positions are designed to reduce pressure on secondary care by offering early, personalised intervention, often requiring skills in behavior change and patient engagement.
