Duties: These coaches work with individuals to set personalised goals, manage stress, build resilience, and develop long-term strategies for mental wellbeing.
Intervention: They deliver high-intensity, time-limited, and often community-based interventions, serving as an alternative to acute, crisis-driven care and aiding in early discharge or reablement.
Collaborative Practice: They work alongside GP practices, mental health professionals, and community partners to support patients with both physical and mental health conditions.
Expertise: Coaches need knowledge regarding factors impacting mental health, risk assessment, and care planning.
