The introduction of a dedicated fertility pathway for endometriosis by NICE is an important step forward.

However, the effectiveness of any pathway depends on accurate diagnosis at the very beginning.

Endometriosis and adenomyosis can often be identified through high-quality transvaginal ultrasound when performed by clinicians with advanced expertise. Yet, this level of training is not consistently available in routine practice, which can lead to missed diagnoses and impact treatment outcomes.

Improving access to expert ultrasound and integrating it into fertility assessments is key to delivering more personalised and effective care.

🧠 Reproductive outcomes depend on accurate diagnosis
A fertility pathway can only be effective if the underlying pathology is identified early.

This requires:

Standardised training in advanced pelvic ultrasound
Greater dissemination of evidence‑based diagnostic criteria
Integration of expert sonographic evaluation into all fertility assessments
Recognition that ultrasound expertise is a core component of reproductive medicine, not an optional add‑on

💡 Implementing the NICE pathway is the first step
Ensuring clinicians are equipped to diagnose endometriosis and adenomyosis before initiating fertility treatment is the essential next move.

🌸 March is Endometriosis Awareness Month 🌸

Today, I’m reflecting on the journey of a young lady who was told egg donation was her only option to start a family. She hadn’t even been given the chance to try stimulation due to concerns about risks like infection or injury to internal organs.

By chance, this couple crossed my path—and with careful preparation, we collected 6 eggs today, 5 of which were mature. It’s still early days, but this is a big step for them.

Egg donation is a wonderful option for building a family, but for young women to hear it as the only path forward without even trying can feel like a hard sentence. Every case deserves individualised care and thoughtful exploration. 💛
