Its Social Prescribing Day!

Knutsford Medical Partnership have 2 Social Prescribing Link Workers, Maria and Denise.

Denise and Maria connect people to activities, groups, and services in our community. They help patients with practical, social and emotional needs that affect their health and wellbeing.

Social Prescribing Link Workers give people time, focusing on ‘what matters to me?’. Denise and Maria work together with our patients to support them to take control of their health and wellbeing.

Social prescribing is an all-age, whole population approach that works particularly well for people who:

· Have one or more long term conditions

· Need support with low level mental health issues

· Are lonely or isolated

· Have complex social needs which affect their wellbeing.

Social prescribing involves understanding the complexities of people’s lives and the inequalities they may face. It can help change the circumstances that make people unwell and empower people to manage existing health problems.
It can help people to connect and to grow in confidence. Social prescribing link workers also support existing community groups to be accessible and sustainable, and help people to start new groups, working collaboratively with all local partners.
