Care Manager, Halifax – Supported Living
An opportunity has arisen for an experienced Care Manager to join our Supported Living service. RHS24 Care provides individual care packages to support and enable people with health needs to remain independent in their homes. Some of the individuals we care for will require practical and emotional support as they come to the end of their lives. We deliver person-centred
and strength-based care to our clients.
As a Care Manager, you will ensure a high standard of care is delivered regularly. You will carry out tasks to ensure RHS24 Care continues to be compliant with the regulatory requirements of the CQC.
We are looking for a candidate who is keen to help set up and develop a service from the ground up with support from the management team. You will support the Registered Manager in the day-to-day management, operation, and administration of the Service.
· Support to develop, promote, and grow the business.
· Setting up new packages of care and ensuring that delivery of service consistently meets the needs of our clients.
· Recruiting staff and sourcing clients for the recruitment part of the business.
· Developing relationships with potential suppliers regarding care packages.
· Maintaining high-quality standards, and ensuring compliance with health and safety regulations.
· Liaising with individuals and their families as needed to ensure that the needs of the people supported are reviewed regularly while developing care plans that continue to meet their needs.
· Holding the on-call phone for emergencies. This is shared with the Care Co-ordinator and Registered Manager.
· Carrying out any other duties that the company deems necessary for the smooth and successful running of the business.
YOU:
· Must have a minimum NVQ Level 3 Health & Social Care Qualification (Level 5 desirable).
· A minimum of 2 years of care management experience within a domiciliary setting.
· Ability to set up and run a project with minimal support from senior management.
· Very good knowledge of CQC rules and regulations and ongoing compliance.
· A full driver’s licence due to the nature of the job.
· Ability to lead and supervise a team within a caring environment.
· Good communication and excellent customer service skills
· Excellent administration skills and proficiency in Microsoft and other computer software.
· Be analytical, pay attention to detail, and ability to work on your initiative.
· Experience in end of life care desirable.

· Salary includes on-call duty pay
· 37.5 hour week (Mon - Fri) and some on-call duty and this is reflected in the generous salary on offer.
