I’m very excited to now be in possession on the Vetscalpel CO2 Laser after putting my order in a the WCVD in Boston at the end of July. Having this fantastic machine will mean I can now treat conditions such as Chronic Pododermatitis and Interdigital Furunculosis ( I have already had good success treating mild solitary cases on one web using my diode laser) much more effectively when large amounts of “false pad” tissue (the cause of the “interdigital cysts”) can be removed with minimum collateral damage and I will be able to use the machine for removing ear lesions/growths and in particular lesions of feline aural cystamosis very effectively and with less collateral damage that I sometimes get with my diode laser. I spent a good 6 hours with Kerry from Vetscalpel at the clinic on 2nd October getting to grips with the machine and doing some wetlab work with it.

I am one for only a very few dermatologists in the UK to have this wonderful tool and look forward to being able to help many animals where this machine maybe the only way of treating their particular issue.

Just a note to let people know that due to exceptional demand, I now have a waiting list for new cases that is close to 10 weeks. There is a shortage of veterinary dermatologists in the Midlands and this has been made worse with the retirement one in a neighbouring city.

I am hoping that this waiting list will fall in the autumn as I know how frustrating it must be for owners at the end of their tether with difficult skin and ear problems in their pets. Unfortunately there is only me and a nurse and we can’t magic extra hours in the day. These skin cases need time spending on them to delve through the previous history and conduct diagnostic tests and therapeutic procedures such as Video Otoscopy and so a second opinion or referral consult can last up to 2 hours.

Your patience is appreciated during this challenging time.

Just to let any past or prospective clients know - over the last couple of months I have seen a large increase in requests for second opinions and also in referrals from other vet practices. This means that there is now a 6-8 week waiting list for new cases depending on the problem and length of consultation required. I can’t extend my opening hours unfortunately so I hope clients understand if they have to wait.

I’m very excited to announce that I have bitten the bullet and decided to invest in a diode laser to use in conjunction with my video otoscopy to allow me to accurately and safely remove tumours and ablate lesions such as polyploid ceruminous gland hyperplasia and other lesions which can be associated with/cause chronic ear infections. The laser energy travels down a small optical fibre, with a 600micron tip, which can pass down the channel of my Storz Video Otoscope allowing me to “zap” lesions with minimal damage to surrounding tissue. Here’s a short video taken showing how the laser has been used to ablate the base of a malignant ear tumour 2 weeks after it was de-bulked and the histopathology report received. Without laser surgery, the only other way of dealing with this would have been to carry out major sharp surgery to remove the bulk of the ear canal or even do a Total Ear Canal Ablation at much greater expense and with much more discomfort for the patient.
I am one of a very small number of vets in the U.K. offering this service.

A bit larger than my usual patient, but same test I do! This month is Allergy Awareness Month. If you pet is suffering with Atopic Dermatitis I can also do an Intradermal Skin Test, like on this Rhino, to decide what allergens to put into a desensitising vaccine.

OPENING UPDATE!

Firstly, I would like to thank all my amazing clients new and existing for being so patient…

Following on from the governments announcements we have taken the decision to re-open. DermVet Leicester has sourced the correct cleaning systems and PPE are in place to protect ourselves and our clients and ensuring we adhere to guidelines.

Some clients may have already had booked appointments before the Covid-19 lock down, so I will be catching up with you over the next couple of days to rearrange. Appointments will be booked in a fair way in relation to your dog or cats’ welfare and their skin/ear condition.

Dermvet will be offering video consultations due to the current COVID-19 situation. It is extremely difficult to provide the high-quality service for owners wanting expert advice and treatment for their pet’s skin or ear problems with the current government guidelines.

Therefore, Dermvet will be providing video consultations to continue to be of great assistance to pet owners needing the services of a veterinary dermatologist.

David will go through the process of a consult. David will look at your pet’s history and then pattern analysis of the skin condition. Considering the visual aspect of the dermatologist examination is still impossible, the specialist knowledge of the veterinary dermatologist can come up with a list of differential diagnoses and can offer the best chance of advising on the correct treatment going forward.

Diagnosis can be confirmed through numerous ways. For example, with a case of otitis, David may ask you to send a swab from the ear canal using a cotton bud. This would then need to be placed into a small polythene bag to prevent it drying out. Once it reaches David, he will be able to check cytology to be sure what pathogens are inside the ear. Similarly, tape strips from the skin, using scotch tape, can also be sent to check the surface skin cytology which can help diagnose yeast infections.

After the video consultation, he can either issue you with a prescription or if you are local or willing to travel to the clinic in Leicester, we can dispense the medication for you.

The RCVS has temporarily relaxed the rules on issuing prescriptions, now allowing them to be issued without a direct examination. It is therefore important that a treatment is prescribed based on the best opinion of what the condition being treated is. Getting advice from a veterinary dermatologist when dealing with skin and ear problems is crucial. Not only for the best diagnosis, but especially considering the RCVS and BVA have asked primary care vets to only see emergency cases only during the Covid-19 restrictions.

Normally I would have caught train from Narborough to Birmingham for this annual meeting, but because if Covid 19, even before the lockdown, it was decided to cancel the meeting and have it as a webinar instead. The coffee is better but do miss catching up with colleagues, discussing cases and seeing the exhibition put on by the pharmaceutical companies.
