The EM3 team presented at this years UHL ACP conference, at Winstanley House!

The day was an annual conference, giving opportunity for all of UHL’s fantastic ACPs to get together and share learning. The day was about celebrating their success, and the growth and future of ACPs at UHL! 🌱πŸ₯

Dr Sunny Jutla, EM3 Lead and Consultant of Emergency Medicine at UHL, gave a presentation focusing the use of simulation in emergency medicine education. πŸ“š

Have a patient presenting with a red, hot and swollen joint that is painful? πŸ˜– Always remember to consider septic arthritis as a differential! 🚨

Remember - in order to cultures to give accurate sensitivities, they should be taken BEFORE empirical antibiotic treatment. πŸ’Š

NEW Lightning Learning ⚑️

Community acquired pneumonia (CAP) is the potentially severe infective consolidation of the lung parenchyma, with the potential to cause sepsis. 🫁

The CURB-65 can be used to help you risk stratify patients and guide decisions on treatment. πŸ’ŠπŸ©»

REMEMBER - if taking samples for cultures and sensitivities, this should be done BEFORE giving antibiotics provided it doesn’t delay treatment. 🚨

Education Wednesday

This Wednesday, we kicked off our weekly simulations with the paediatric EM team. They simulated a scenario of a 2 year old presenting with reduced consciousness. On A-E assessment, they were found to be hypoglycaemic. πŸ˜₯ They were also treated for sepsis.

The team remained calm under pressure and worked well together to navigate this scenario to recognise, stabilise and treat this patient. βœ…

The debrief afterwards gave great opportunity for reflection and discussion around key learning points. 🧠

REMEMBER - we run departmental simulations every Wednesday morning, which can be booked via our website!

FY2 ED Teaching πŸ“š

Yesterday, FY2 doctors at LRI Emergency Department took part in a peer-to-peer teaching session, covering chest pain syndromes, dyspnoea and ABG interpretation. πŸš¨πŸ’‰

The turnout was fantastic and there was lots of great learning resources shared between colleagues. They used gamification to make the learning fun, interactive, and to bring the team together! πŸ€—

Aortic dissection is a medical emergency requiring urgent diagnosis and treatment to reduce mortality and morbidity. πŸ«€

This Teaching in Ten series guides you on when to suspect, how to investigate, and how to manage aortic dissection in the emergency department. πŸš¨πŸš‘

The LRI Physician Response Unit (PRU) attends East Midlands Ambulance Service () calls of all severity, aiming to deliver the right care, for the right patients, in the right place, at the right time. πŸš‘

Our registrars joining the PRU team undergo a 10 day course to cover essential knowledge and practical skills, enabling them to manage a variety of clinical presentations in the pre-hospital environment. 🏑

The training includes common (and uncommon!) scenarios and simulations, including both medical and trauma presentations for both adults and paediatric patients. 🚨

Check out this reel to see some of the training in action! πŸ‘€

Measles is an extremely infectious disease, and we are seeing incidence rise across the nation and within the paediatric emergency department. πŸ₯πŸš‘

Use this lightning learning to keep your knowledge up-to-date and ensure you spot measles. Early detection not only allows isolation and reduced transmission risk, but reduces the risk of complications for the patient. πŸ€’

Our most recent blog, β€œA Guide To Less Than Full Time Training”, is produced by Dr Molly Cartagna, currently a foundation trainee doctor working in the UHL paediatric ED. πŸ₯🚨

In this blog, she discusses her discussion to apply for less than full time (LTFT) training, and how this has helped her ensure she can work sustainably throughout her career. πŸ”‹

If you are a doctor considering LTFT training, take a look at this guide to gain insight into how this may help you in your career. πŸ’™

Sepsis Teaching in πŸ”Ÿ

Sepsis is a life-threatening illness caused by the body’s response to infection. In the UK alone, 5 people die every hour from sepsis. 🩸

This sim-based Teaching in Ten allows you to test, apply and supplement your knowledge on sepsis. For all patients who are acutely unwell, just ask β€œcould this be sepsis?”. It could save a life. βœ…

Initiate the sepsis six bundle within one hour of identification, and escalate early to seniors. ☎️

We recently shared our Lightning Learning on Acute Behavioural Disturbance - now we have a sim video to go with it! πŸ₯³

Notice how the staff in this video use their communication techniques to try to de-escalate the situation, and only call security to restrain the patient when their behaviour is a safety concern. 🚨

Remember - aggression often signals unmet needs, and might indicate a delirium. Try to identify and meet these needs as soon as possible! 🧠

There is a brand new Acute Behavioural Disturbance guideline for use in UHL hospitals, including guidance on rapid tranquillisation, at the end of this video. You can use this as a learning aid, however please remember to follow local guidelines for the hospital you work in. πŸ₯πŸ“š
