Leighton-Linslade Health Connections Primary Care Network for Leighton Buzzard - LLHC PCN We are the Primary Care Network for Leighton Buzzard, we work in coordination with all three GP practices.

Formally known as The Leighton Buzzard PCN, now Leighton Linslade Health Connections (LLHC) we offer various primary care support roles to help our patients take ownership of their healthcare and connect them to their community. We work on the Personalised Care Model – and work with you to help you achieve your own goals, at your pace and by focusing on what’s most important to you. Our service information is also listed on our linked GP Practice websites.
