Boultham Park Medical Practice

Boultham Park Medical Practice GP Practice Patients who visit a GP for a musculoskeletal (MSK) problem will instead have an appointment with an FCP physiotherapist.

The doctors and staff at Boultham Park Medical Practice are proud to offer the highest standard of patient-centred healthcare; this was recognised back in 2011 when the surgery was awarded the Quality Practice Award by the Royal College of Practitioners. The quality of the service we provide was further confirmed following an Inspection by the Care Quality Commission in May 2016 when we were graded good overall and in each individual area of the inspection. We provide a wide range of Primary Care service, typical of what you should expect from all GP surgeries, and run many clinics for the management of Chronic Diseases such as Asthma, COPD and Diabetes. We have other local services such as the Midwife and a Mental Health Practitioner working out of the surgery and we work closely with our health colleagues across the city. We also have a First Contact Practitioner (FCP) who is a physiotherapist professionally qualified to treat patients without a referral from a GP or other healthcare professional. Primary Care Networks have recently been introduced to help primary care providers to work more collaboratively and at scale. We are part of the Apex Primary Care Network which consists of 5 local GP Practices with a collective patient list size of approximately 52000 patients. We are also part of the Optimus Group of GP practices. These are other GP surgeries in our locality with whom we work closely across a number of areas; in particular in offering improved access for appointments outside of our normal core hours. Boultham Park Medical Practice is also a Training Practice. This involves training the doctors and nurses who have chosen to specialise in primary care and who we all will need to look after us in the future!

I am pleased to inform you that Boultham Park Medical Practice has been awarded a 5-Star Quality award from the Lincolnshire ICB Primary Care Quality Team (PCQT)

The QEWS (Quality Early Warning Score) tool not only helps practices highlight the quality they currently provide but also supports their future quality aspirations in various domains such as Safe, Effective, Responsive, Caring, Equitable, Well Led and Sustainable.

QEWS also identifies areas where quality improvement may be needed. In such cases, the PCQT will actively support and guide those practices to achieve a QEWS star rating that reflects the National Quality Boards (NQB) quality expectations.

I'm sure you will all agree this is a well deserved award and join us in congratulating the Boultham Park Medical Practice team.

Carers Rights Day is taking place on Thursday 20 November.
Here at Boultham Park Medical Practice we’re proud to join Carers UK and organisations from across the country in raising awareness of the rights, entitlements and wider support available for unpaid carers.

This year’s theme ‘Know your rights, use your rights’ is all about making sure more carers understand their entitlements and feel confident to use them. We’re encouraging everyone to get involved and to find out how carers rights can help you to:
✔ Access support
✔ Juggle care and work
✔ Look after your health and wellbeing
✔ When the person you care for is coming out of hospital Find out more, and get involved in Carers Rights Day at https://go.carersuk.org/CRD

Are you a Veteran living in Lincoln?

To ensure you receive the care and support you’re entitled to, make sure your GP practice is aware of your Veteran status and it is correctly recorded on your medical records.

Why is this important?
If your condition is linked to your military service, your referral can be marked accordingly — ensuring your care is prioritised where appropriate. This does NOT mean priority GP appointments.

Support for Veterans:
There are over 110 organisations offering support to the armed forces community!
You can find them on the Connect to Support website under the Veteran Support Directory. https://lincolnshire.connecttosupport.org/armed-forces-community-directory/

Please share this with any veterans you know — let’s make sure every veteran gets the support they deserve!
