Job description
Job Summary:
We are seeking a dedicated and experienced Clinical director Nurse to join our multidisciplinary team. The successful candidate will take a leadership role in the coordination, delivery, and monitoring of complex care packages for patients, ensuring high-quality, patient-centered care across the bsuiness and within the community where appropriate.
Key Responsibilities:Leadership & Management:
* Provide clinical leadership and supervision to nursing and support staff.
* Oversee the implementation and review of individualized care packages.
* Act as the main point of contact for complex patient cases requiring multidisciplinary coordination.
* Ensure compliance with regulatory and clinical governance standards.
* Contribute to the development of clinic policies, protocols, and best practices.
Care Coordination:
* Lead the assessment, planning, and evaluation of care packages tailored to individual patient needs.
* Liaise with GPs, social workers, allied health professionals, and external providers to ensure continuity and consistency of care.
* Monitor outcomes and update care plans in response to changing health needs.
* Ensure timely referrals and access to appropriate services and interventions.
Clinical Duties:
* Perform and oversee clinical assessments, wound care, medication administration, and chronic disease management.
* Support the management of patients with long-term conditions and complex needs.
* Maintain accurate and up-to-date clinical records using appropriate electronic systems.
* Audit external users work to ensure the quality is meeting all requirements
Training & Development:
* Mentor and support junior staff and students.
* Organise and deliver training sessions related to care planning and coordination.
* Keep up to date with current best practices, legislation, and clinical guidelines
Essential Criteria:
* Registered Nurse with a valid NMC registration.
* Minimum of 8 years of nursing experience, preferably in a community or primary care setting.
* Proven leadership and team management skills.
* Strong understanding of care planning, safeguarding, and multi-agency working.
* Excellent communication, organizational, and IT skills.
* Work with CCG,ICB, LA and MDTs
Desirable:
* Previous experience coordinating care packages or working with complex patients.
* Experience in case management
* Qualification or training in chronic disease management, community nursing, or leadership.
* Knowledge of local health and social care systems and pathways.

Hello all, I hope we are all well and not feeling the hump day too much! Friday is right around the corner 🥳

I just wanted to create a post to spread the June awareness of PTSD month. Post-traumatic stress disorder can appear in various signs and symptoms, affacting (all) aspects of daily living. Any situation can cause PTSD and there is no time limit on the recovery period.
The awareness of PTSD is important and the understanding of what this mental condition is/can do. The world health organisation (WHO) estimates around 3.9% of the world’s population has experienced PTSD in their life, and around 50%-70% of people in the uk will experience PTSD in their life!!
For anyone who is wanting to find out more information or join in on any of the PTSD campaigns, then I urge you to become involved. You can find out more information by visiting the PTSDuk website, Mind or NHS website, to start making your difference today.

My story...

He’s 12, he’s your baby brother, and you get the call to say he’s been hit by a car and has passed away.

That was my life on October 9th 2009.

My brother was able to be revived but had an extensive brain injury.

The next 18 months were a blur of hospital stays all over the UK.
