This we're proud to highlight the importance of nurses, like Catherine, within diabetes care. A former Diabetes Specialist Nurse, who now works as a Diabetes Young Adult Transition Coordinator in Northern Ireland, Catherine is helping young adults to manage their diabetes over time.
“Young people are the future… and I want them to know they are seen, valued and cared for.”
Meet Catherine. A former Diabetes Specialist Nurse, who now works as a Diabetes Young Adult Transition Coordinator in Northern Ireland - helping young adults to manage their diabetes over time.
“I aim to meet young people at around 15 and walk alongside them through their transition to adult services until around age 25. I send appointment reminders, offer support and reach out after any hospital admissions.
“I’m at the end of a text or call - diabetes-related or not.”
For Catherine, the small moments matter just as much as the big ones. She explains to us: “getting to see them achieve their goals – that’s the greatest reward. Remembering to take insulin before meals, heading to university, or travelling the world. Each one is possible, and each one matters.
“A diabetes diagnosis doesn’t take away your ability to live out your dreams. You can achieve things others never believed possible - and we’ll be right there cheering you on.”
