A very warm welcome to our new team members! 💚

🌈Practice Announcement 🌈

We are pleased to announce that we have 2 new GP's starting with us this month!

Dr Faisal Rahim started last week and will be working Thursday and Friday.

Dr Joe Bishop is joining us on 19th August and will be working Monday and Tuesday.

We also have a new final year GP trainee, Dr Kyle Millar, who started with us last week. He is here for 12 months in order to complete his training and become a fully qualified GP.

In addition to this Dr Julie Gupta has also increased her days to include Fridays, and we are hoping this will try and give more availability for GP appointments.

They are a valued addition to our team and we hope you all join us in welcoming them to Stonefield Surgery!

🩵𝗣𝗲𝗻𝗻𝗶𝗻𝗲𝘀 𝗣𝗖𝗡 𝗠𝗲𝗻𝘁𝗮𝗹 𝗛𝗲𝗮𝗹𝘁𝗵 𝗟𝗶𝘃𝗶𝗻𝗴 𝗪𝗲𝗹𝗹 𝗦𝗲𝗿𝘃𝗶𝗰𝗲🩵

Towards the end of 2023, Pennines PCN collaborated with Pennine Care to implement the new Mental Health Living Well Service, offering easy access to an array of specialised mental health professionals for patients of our community.

Over the past 9 months, the Mental Health Living Well service has grown from strength to strength, and a big part of its success is down to our Senior Mental Health Practitioner, Jake.

Jake, alongside his fabulous team, has worked incredibly hard with all of our 6 GP Practices to implement this new structure into Primary Care, and has been a driving force behind it's success.

We are very grateful to have Jake and the team supporting us and our patients!

We're so proud of Rochdale nurse Jake Riley, who's been crowned the borough's clinical champion in recognition of his outstanding contribution to transforming community mental health care.

𝗚𝗣 𝗣𝗔𝗧𝗜𝗘𝗡𝗧 𝗦𝗨𝗥𝗩𝗘𝗬 𝗥𝗘𝗦𝗨𝗟𝗧𝗦 👏

Pennines PCN collective results 2024 results based on % of patients saying their overall experience of their GP Practice was 'good'.

A huge well done to our individual GP Practice teams within Pennines PCN. Collectively we have achieved 3rd place out of all Greater Manchester PCN's for 'Overall Experience'.

As a PCN and as individual GP Practices, we will always work hard towards improving the services that we provide.

The exercise referral programme is a 12 weeks long and an opportunity to take part in a range of activities at Your Trust leisure centres across Rochdale borough, have support from the specialist health and wellbeing team throughout, and gain advice on continuing your healthier lifestyle.

The exercise referral team are highly qualified instructors and will meet with you to motivate you every step of the way through the programme.
