Liverpool Diabetes Partnership Your diabetes service in the heart of Liverpool in collaboration with Mersey Care NHSFT and Liverpool Hospitals.

We provide education and support for the prevention and treatment for diabetes in Liverpool. Follow along for awareness, education & support. Liverpool’s new approach to diabetes care moves the focus from hospital-based care to community-based clinics that will promote good self-management, integrated delivery and education. Diabetes is one of the biggest challenges facing the NHS and is a major c

ause of death. Estimates suggest that 8.3% of people in Liverpool may develop diabetes by 2030 unless more support is provided to help people take better care of their health and wellbeing. The new service has been designed to tackle this challenge head-on, with a shared vision to improve outcomes for people living with diabetes. It forms part of the Healthy Liverpool programme, which aims to transform healthcare in the city so that everyone receives “consistent, high-quality healthcare” that will lead to “longer, healthier lives” by 2020.

It’s – let’s talk about the 9 key care processes!

These vital checks help detect complications early, reduce long-term health risks, and support people with diabetes to live healthier lives. Everyone with diabetes should receive all 9 checks at least once a year.

What are the 9 Key Care Processes?

1. Urine Albumin-to-Creatinine Ratio (UACR)
A yearly urine test to spot early kidney damage & heart risk. Maintaining good blood pressure & glucose levels, staying active & eating well will help protect your kidneys.

2. HbA1c
A blood test showing average blood glucose over 2–3 months. Most people aim for around 7% (53 mmol/mol), but this should be individualised.
