🤝 Community Connectors - helping you discover your community and new hobbies whilst supporting you with social isolation

👂Strand By Me - our hub connecting you with our community partners, as well as directing you to health and wellbeing services to support you. All face to face.

👥️Social Prescribing Link Workers - social Prescribing can help you address mental health issues and other things that may be affecting your wellbeing. Your Social Prescriber will work 121 with you and use a person centred approach to help you improve your wellbeing and confidence
