Wedi'i gynnal gan Dîm Canser Cenedlaethol GIG Cymru, mewn partneriaeth ag Ymchwil Canser y DU (CRUK), bydd y digwyddiad addysgol rhad ac am ddim hwn yn darparu llwyfan gwerthfawr i rannu'r diweddariadau diweddaraf mewn diagnosis canser, llwybrau atgyfeirio a gofal claf.

Primary Care Cancer Education Event

Hosted by the NHS Wales National Cancer Team, in partnership with Cancer Research UK (CRUK), this free educational event will provide a valuable platform to share the latest updates in cancer diagnosis, referral pathways, and patient care.

The Primary and Community Care Academy in collaboration with Primary Care presents Lunch and Learn – Access Standards Update.

This 30-minute session will consist of a 20-minute presentation from contracting. The session will delve into access standards and give 2025 updates.

Signposting to links to useful resources will also be shared.

Attendees will also get the opportunity to ask contracting questions

Mae'r Academi Gofal Sylfaenol a Chymunedol mewn cydweithrediad â Gofal Sylfaenol yn cyflwyno Cinio a Dysgu – Tîm Thrombosis Gwythiennau Dwfn (DVT)

Bydd y sesiwn 30 munud hon yn cynnwys cyflwyniad 20 munud gan Emma Roberts, Nyrs Arbenigol Thromboproffylacsis ar gyfer y tîm canolog a'r tîm DVT Canolog. Bydd y tîm yn cyflwyno eu hunain, yr hyn maen nhw'n ei gynnig, sut i gyfeirio a sut mae'r tîm yn cefnogi Meddygfeydd Teulu.

Bydd cyfle i'r rhai sy'n mynychu gofyn cwestiynau i'r Tîm DVT mewn sesiwn Holi ac Ateb fer ar ddiwedd y sesiwn.

The Primary and Community Care Academy in collaboration with Primary Care presents Lunch and Learn – Deep Vein Thrombosis (DVT) Team

This 30-minute session will consist of a 20-minute presentation from Emma Roberts, Thromboprophylaxis Specialist Nurse for central and the Central DVT Team. The team will introduce themselves, what they offer, how to refer and how the team support GP Practices.

Attendees will have the opportunity to ask the DVT Team questions in a short Q&A at the end of the session.

Taith Liane drwy Ofal Sylfaenol

Symudodd Liane i ofal iechyd o addysg yn 2018 a dechreuodd fel Codydd Clinigol mewn meddygfa. Gyda chefnogaeth y practis, hyfforddodd Liane fel fflebotomydd gyda'r bwriad o adleoli yn ystod covid. Yn dilyn y pandemig, roedd Liane wedi mwynhau'r rôl wynebu cleifion gymaint nes iddi hyfforddi fel Gweithiwr Cymorth Gofal Iechyd ac ennill Tystysgrif Lefel 3 wrth weithio yn yr un practis.

Yn 2021 cafodd ei derbyn ar y Dystysgrif Lefel 4 mewn Ymarfer Gofal Iechyd, a welodd hi'n cael ei rhyddhau un diwrnod yr wythnos i fynychu'r coleg ochr yn ochr â gwaith. Cefnogodd y bwrdd iechyd Liane i gwblhau gradd nyrsio ran-amser gyda'r Brifysgol Agored a gwblhaodd ym mis Mehefin.

Rydym yn falch o rannu bod Liane wedi derbyn ei PHIN a gall alw ei hun yn swyddogol yn nyrs gofrestredig - Llongyfarchiadau📣

Liane's journey through Primary Care

Liane moved into healthcare from education in 2018 and started as a Clinical Coder at a GP surgery. With the support of the practice, Liane trained as a phlebotomist with a view to redeployment during covid. Following the pandemic Liane had enjoyed the patient facing role so much she trained as a Healthcare Support Worker and gained a Level 3 Certificate whilst working at the same practice.

In 2021 she was accepted onto the Level 4 Certificate in Healthcare Practice, which saw her released one day a week to attend college alongside work. The health board supported Liane to complete a part-time nursing degree with the Open University which she completed in June.

We're pleased to share that Liane has received her PIN and can officially call herself a registered nurse – Congratulations📣
