Better Physiotherapy is a physiotherapy clinic in Loanhead, Midlothian. We always complete a comprehensive assessment before using a range of specific treatment and management strategies to address the cause of your pain or problems. This would usually include education, hands on therapy and a tailored exercise programme. Our physiotherapists have a wide variety of experience and expertise from a variety of settings including clinical, occupational, sporting and further education. We held our 1st inservice of the year last week and it was all about pain around the knee cap. This problem is sometimes called "runner’s knee", and is the common type of knee pain that we would see. Although at times it can be pretty disabling it is normally very treatable.

The main points we took away from the inservice were that:

- Age, weight, height, hip angle, and knee “noise” are not risk factors for developing this problem
- A lack of quads strength may be a risk factor
- It does not tend to get better on it’s own
- A combination of education, strengthening, and possibly movement retraining tend to help most in the longer term
- Sometimes taping/braces and insoles can help, these may have more of a role as a short term solution (like using a crutch or ankle support)
- It can take 3 months or more for the problem to get better with the right management

Welcome Rabia! 👋

We are delighted to now have physiotherapist Rabia Rauf working at the clinic on a Tuesday and Wednesday afternoon and evening. Rabia has over 8 years experience in a variety of countries and settings, and has recently completed a Masters in Advanced Physiotherapy Practice at Glasgow Caledonian University 👩‍🎓

Rabia is passionate about helping clients regain their independence and getting back to enjoying their life. As part of this she enjoys working closely with clients, using tailored evidence based treatments to help them achieve successful outcomes.

Rabia also loves exploring the Scottish outdoors ⛰, learning about new cultures 🇵🇰🏴󠁧󠁢󠁳󠁣󠁴󠁿🇳🇴 and cuisines 👩🏾‍🍳.

From tomorrow (Monday), the northbound carriageway of Straiton Road (towards the City Bypass) in Loanhead will be closed to vehicles from 6pm to 6am for approximately two weeks.
The road will be closed in two phases for the resurfacing work:
• Phase 1: from the roundabout on Straiton Road (next to Sainsbury’s) to the junction into Straiton Park and Ride
• Phase 2: from its junction with Straiton Park and Ride to the*roundabout where it joins with A720 bypass
Buses will still be able to access Straiton Park and Ride.
A diversion will be in place.

We are all probably familiar with the advice for acute injury management of (P)RICE. The aims of this advice are to protect injured tissue from further injury, reduce swelling & bleeding, alleviate pain & allow for quick healing. Improvements in our understanding of injury, healing, and best management have changed what we understand as best advice. It seems we should take a slightly “groovier” approach ✌️

Immediately after injury-

* Protection- Restrict movement & activity for a few days. Use pain as a guide. Some mild discomfort that settles quickly is generally ok.
* Elevation- When resting try and keep the affected area at heart height. This helps fluid to drain from the injury. The evidence for this is not strong but it’s unlikely to cause harm.
* Avoid anti-inflammatories- Inflammation is one of the early stages of healing. Reducing inflammation may have a negative effect on the healing process and timescale. There is little evidence for ice in treating injuries. Ice can be used to help with pain relief but avoid temperatures below 0 deg and for periods of more than 10 mins, as again this may have a negative effect on healing tissue.
* Compression- Use compression such as a tubigrip/compression tape to reduce excessive tissue swelling without restricting comfortable movement
* Education- Give advice on the above. Encourage an active recovery strategy is best and reassure that the body does a great job of repairing itself when given the right conditions and time.

After few days-

* Load- Begin to gradually increase normal movement & exercise. Again use pain and comfort as a guide.
* Optimism- The brain and our outlook play a huge part in our recovery. Most tissue injuries heal very well and a realistic but confident outlook will reduce the likelihood of the problem becoming chronic and disabling
* Vascularisation- healing tissue needs blood and oxygen and a great way of increasing this is with cardiovascular exercise. It is also great for your physical and mental health generally
* Exercise- the best way of restoring normal movement, strength, control and function is by exercising the injured area. Again pain can be used as a guide.
