Provide our customers of all ages with support and counselling in order to guide individuals towards living happy and fulfilled lives, whether to independence, end of life or health recovery. Empower individuals through encouraging the active participation in their care plans and support plan reviews. Provide individuals with relevant information through, one-to-one sessions, informative literature and carry out interactive discussions, in order to help individuals’, mature, understand what is happening around them and how it make impact on them
Ensure continuity of care, support and guidance within all aspects of their service requirements
Prepare young people to transition into successful adults and help adults in various life aspects. Support individuals with their desire to be productive /active members of society as required i.e. continue their education, seek new employment opportunities, participate in community project or groups.
