Though most patients will reach this stage of imperceptible surgery signs within three weeks, we find that subtle improvements continue for some weeks past this stage, as minor swelling subsides and the eyelid skin becomes even smoother.

Whatever our age, concerns or condition, a healthy and natural appearance of our eyes has again and again been shown to have a strongly positively effect on our quality of life.

We train lots of residents and junior eye surgeons every year and I always make it a personal point to try and explain what is so special about a career in oculoplastics, as it is not always self-evident.

I have never been able to put it as eloquently as the loving mother of this 5-year old patient of mine from several years back.

For this reason, I felt that this was a card worth sharing..

Happy outcome eyelid surgery has got to be all about the 𝘚𝘶𝘣𝘵𝘭𝘦 things.

Designing each eye’s sculpting independently, so as to deliver a good result with as close as possible symmetry (absolute symmetry is not really possible nor really desirable).

This upper blepharoplasty case highlights a few subtle design tweaks:

✅ Different design in the skin, muscle and fat sculpting between R and L eye

✅ Subtle ptosis repair

✅Brow stabilization

✅Left medial fat pad transposition to fill a hollow

Surgical repair of non-severe drooping eyelid (ptosis) cases in young children is occasionally delayed, so that accurate outcomes can be achieved when the individuals are older.

There is always a great argument for not delaying this treatment any longer than early adulthood.

This patient’s comment clearly demonstrates the benefit that this treatment can bring about.

Congenital eyelid droop (ptosis) is different to adult-onset ptosis where the eyelid muscle slips due to overuse.

In congenital eyelid droop, the levator muscle is poorly developed and instead replaced by rigid strands of fat and scar tissue in the place of normal flexible healthy muscle fibres.

Surgically achieving perfect symmetry along with good eyelid movement and the ability to close the eye safely is therefore much trickier to get right.

This patient of ours, a 24-year old professional athlete, visited us with a history of previous unsuccessful surgery.

I carried out revision left upper congenital ptosis repair by anterior levator resection.

At two months following surgery (bottom image), his eye surface is healthy and one can still appreciate the minimal intentional overcorrection and and fading incision line (both of which will correct within the next few months).

In contrast to upper eyelid hooding due to skin excess over the eyelid, Ptosis is a condition when the entire eyelid structure droops due to damage of the muscular structure which keeps our eyes open.

Ptosis can present without any skin excess (like in our patient here who did not require blepharoplasty) or along with skin hooding.

Assessment of the size of the eyes and ptosis should always be carried out as part of your blepharoplasty consultation because missing a ptosis will result in an unsatisfactory blepharoplasty outcome.

The majority of our male patients express one main surgical wish: 𝙨𝙪𝙗𝙩𝙡𝙚𝙩𝙮.

Technically, this means a few things, but mostly it ‘s about:
•Completely scarless surgery,
•Staying true to age
•Avoiding feminisation of the brows.

For this reason, brow lifting is often not desirable unless the position is very low.

Here is an example of natural upper and lower blepharoplasty, before surgery and at the one-week review.

Our patient also suffered watery eyes due to lower eyelid laxity and this was simultaneously addressed with lateral canthal tightening leading to full resolution of these symptoms.

Over the following couple of weeks, any residual bruising and swelling will also settle.

Do standby for his final images when he flies back to London for his final follow up.

The textbook approach to a case like this, where there is significant drooping of the brow, is to combine brow lift surgery with upper blepharoplasty.

The idea is that with a blepharoplasty alone, only removing the amount of skin needed to tidy the eyelid would otherwise further pull the brow down.

In the real world however not all patients want, or can have a brow lift and those cases require fine balancing.

This patient of ours was seeking minimal surgery, quick downtime and no visible incision lines for any length of time.

I loved this case as it nicely highlights the power of blepharoplasty in natural facial rejuvenation and functional improvement.

Though nothing is ever guaranteed in surgery, the expectation really should be that the blepharoplasty scar will disappear.

If during your assessment there is suspicion that this may not happen, surgery should be avoided.

Us surgeons should be doing more to show the progress of these scars, because they do not disappear in 2 weeks.

We will cover the recipe for an invisible scar at a later video but in short there are 2 ingredients: precise surgery & attentive to instructions patients. Both equally crucial.

Here is a fresh example of an upper blepharoplasty incision at 1 week and then at 4 weeks.

Final recovery image to follow in a few months.
