Upper and lower eyelid blepharoplasty ✨

This lady was unhappy with the appearance of her upper eyelids due to the skin laxity and skin excess as well as the contours of the lower eyelids and the fact that it was giving her a more tired appearance.

She underwent upper and lower eyelid blepharoplasty which included a lateral canthopexy.

For the upper eyelids, the intervention involved reducing the skin excess and some of the volume in the inner corner of the upper eyelids.

For the lower eyelids the procedure scraps up included redraping of the fatty tissue from the lower eyelids over the rim of the orbit to produce a softer transition between the cheek and the lower eyelids, reduction of the lower eyelid skin excess through an access incision under the lashes and lateral support for the lower eyelids to protect the almond shape of the eyes. 🤩

Building Projection For The Nasal Bridge…

When it comes to boosting nasal bridge projection, you've got choices! You can opt for natural tissues like costal cartilage or facial grafts, or go for synthetic materials. The key is finding what suits you best! 🌟

Now, if you choose coastal cartilage, you have different techniques at your disposal. You can lay it over the top of the bridge for a subtle enhancement, or you can create a structured, parallel design that resembles walls going up.

We lovingly call this approach "elevated spirographs" because they're wedged on each side of the nasal septum and supported by the nasal bones. It's a technique that can produce stunning dorsal aesthetic lines, giving you that beautifully defined look! 💪

Part three in our series of how to manage undereye circles and bags…

Achieving a youthful appearance requires a front cheek contour that looks flat when viewed from the profile. But what happens when smoothing this contour, and blending the delicate junction between the lid and cheek with volume augmentation (think fat transfer or filler), results in an over-the-top cheek volume? 🤔

That's where a lower eyelid blepharoplasty comes into play. It can involve re-draping some of the fatty tissue, and our trusty 3D assessment plays a crucial role in this journey. 🌟

Volumes Under The Eyes…

In our previous video, we talked about managing eyelid bags and how the volume plays a pivotal role in achieving the right look. It's like sculpting, but instead of clay, we're working with shadows. When you're trying to banish a shadow caused by a valley, you need to figure out how much volume is just right to level it out. 🏞️

Now, here's the thing - you can think about this in 2D photos, but it's even easier in 3D. You can reference the volume, keep building it up, and then look from the side view. This helps you decide if it's just right or if it's ventured a tad too far. If it's gone too far, don't worry - there are better treatments! It's all about finding your perfect balance. 🌟

Revision rhinoplasty often calls for structural support and a tailored approach.

This patient underwent revision rhinoplasty with a rib graft, following two previous attempts elsewhere. Now two months post-operative, they report no bruising, a smooth recovery, and a significant improvement in both structure and feel.

In complex cases, cartilage grafting, particularly from the rib, can offer the necessary support and stability to restore definition and balance. Some swelling will continue to resolve in the months ahead, but we’re pleased to see early satisfaction with both the form and function of the result.

Contours under the eyes…

Eyelid bags are a problem that bothers a lot of people, and it can start quite early in life. Effectively this is a 3-D problem, but the assessment is usually done on the front view. So it's 2D thinking for a 3D problem which can create issues.

When we aim to fix what we see from the front, we might unintentionally create convexity - like adding fullness. It's a bit like what can happen with breast implants, creating upper quadrant fullness when the natural shape tends to be flatter. Why? Because gravity has its say, influencing not just our cheeks but also other parts of our body. It's all about finding the right balance! 🌠

Recovery from neck lift surgery…

After the surgery, it's inevitable to experience some tightness under your chin. It's completely normal and manageable with the right medication. It also clears relatively quickly after surgery, and you’ll be supported throughout the process. 💪

But remember, recovery isn't just about medication. Mobility is also very important. Don't hesitate to move around while wearing your support garment, it will help your recovery to be smoother and quicker. 😌💫
