Welcome to Dr Silberstein Clinic! Sarah Reid just joined the clinic and will be offering podiatry services.

Sarah studied podiatric medicine at the University of East London and is currently working as a Senior Podiatrist at North Middlesex Trust in Enfield providing a range of podiatric treatments.

Since 2020, Sarah has been offering home visits in the North West London area.
Sarah carries out routine podiatry, minor ops nail surgery and Biomechanics (orthotics and exercises).
She provides thorough personalised treatments and takes a holistic approach to her work.

Feeding with Flat or Inverted ni***es
It is, with some practice, mostly possible to breastfeed as normal. It is a good idea to get an experienced Lactation Consultant to help with positioning and attachment. Breast shaping can help latching on deeply (please swipe for illustrations)
Avoid engorgement, as this can make the ni**le even flatter. Feed frequently, use lymphatic massage and cold compresses to reduce swelling.
Use gentle massage on the ni**le, hand expression, an electric pump or a ni**le everter to bring out the ni**le before feeding.

If baby really can’t latch, try a ni**le shield, under the guidance of a Lactation Consultant
Flat and inverted ni***es will often protrude more with time and with frequent use. If direct feeding is not possible consider pumping (it might just be one breast)

Colostrum collection

Today I had a lovely first time mum in my clinic for an antenatal appointment. She is now 39 weeks pregnant and very anxious to breastfeed. We spent time talking about things a mum can do to prepare herself for breastfeeding after birth. One of the important things a pregnant woman can do is colostrum harvesting. I showed my patient the technique and I was able to put her mind at ease: I helped her to express 4 full syringes in about 15 minutes. She was so relieved and excited.
Harvesting colostrum is something I regularly recommend to my patients from 36/37 weeks *. It’s great to have colostrum ready for those early hours and days after birth as a back up. It’s also really good to be familiar with the technique, in case expressing is needed after baby is born.

*If there is any doubt if harvesting colostrum is suitable for you, please ask your Obstetrician, Midwife or GP.

This baby was seen today at my clinic. Very little lift in the mid section of the tongue but really good protrusion (outwards movement). This baby is spluttering, coughing and dribbling on the bottle and has a lot of symptoms for reflux. She can’t sleep well and barely naps in the day. Immediately after the Tongue tie division we saw the bottle feeding improve, much less clicking/gulping and coughing observed. I am sure this will make a difference to the amount of air baby swallows during feeds and ultimately also to the reflux symptoms.

A lot of my patients want to know which pumps I recommend.
This very much depends on what the pump is intended for.
For situations where a very efficient and time saving option is needed (for example low milk supply, for inducing lactation, relactation) I recommend using a double electric hospital grade pump. These two are my go-to pumps, which are tried and tested. They work very well and are competitively priced at £159 for the Spectra Pump and £179 for the Baby Buddha Pump.
When buying a pump it is very important to pay attention to the fl**ge size, a well fitting fl**ge will not only be more comfortable but can also have a big impact on the amount of breastmilk pumped.

Breastfeeding Support Groups available at Dr Silberstein Clinic. Mornings 10-12am.

Group bookings with Lactation Consultants or Breastfeeding specialists (Breastfeeding Counsellors and Peer Supporters) are run with a maximum of 2 clients per breastfeeding specialist. You will receive support for your breastfeeding issues. Group support sessions are excellent to assist basics like positioning and latch, basic information about normal breastfeeding/pumping strategies, normal baby feeding patterns, normal baby weight gain, breast and ni**le care, and ways to maintain and protect your milk supply.
