Breaking Barriers in the Independent Sector

ASiT is actively working with the Confederation of British Surgeons (CBS) and BOTA and independent sector providers to tackle one of the most persistent challenges in surgical training: access to cases in the independent sector.

Together, we’re developing innovative, collaborative solutions to ease systemic barriers and ensure trainees in craft specialties can gain the operative experience they need.

This is partnership with purpose - driving practical change to unlock training opportunities across all sectors.

A Clear Path to Escalate Training Concerns

ASiT is working collaboratively with BOTA and the JCST CST Advisory Committee to develop a stepwise escalation framework for CST trainees.

This framework uses JCST Quality Indicators and ARCP requirements as benchmarks to guide trainees through a clear, appropriate escalation route when raising concerns about training quality, supervision, or rota-related issues.

Our goal is simple:
👉 To ensure trainees know how, when, and where to escalate concerns - and to make sure those concerns lead to action.

The True Investment in Surgical Training

Every surgical trainee represents a significant national investment.
In England alone, a host Trust receives:
• £13,933 per trainee in placement fees
• £30,856 in average salary contribution

When scaled to the UK’s estimated 7,471 surgical trainees, this amounts to an annual government investment of ~£334 million.

That’s £334 million of taxpayer funding intended for training.

But intention doesn’t equal impact.
We need greater accountability to ensure that the money allocated for training is actually used to deliver training. This is not simply a call for more funding; it’s a call to reappropriate existing budgets so they serve their true purpose.

It’s time to shift the narrative.
Training budgets must train.
Trainees and patients deserve nothing less.

Championing Change in Surgical Training

ASiT has been at the heart of the Medical Training Review (MTR) listening phase, contributing to every steering group and focus group to ensure the trainee voice was front and centre.

Thanks to sustained advocacy from ASiT and key surgical stakeholders, the MTR report now includes a specific recommendation that craft specialties must have access to the independent sector for training; a major step forward in protecting high-quality, hands-on surgical experience for trainees.

This is what collective advocacy can achieve.
And we’re just getting started.
