🤝Working Together to Strengthen Care — A Morning of Learning at Carlton Court✨
On the morning of July 18th, Carlton Court welcomed a different kind of visitor — not just guests, but partners in care.
We were honoured to spend time with Linda Jernberg-Collins, Nurse Consultant with the Hospital at Home Team, and her colleague Tanya, who delivered a meaningful and insightful session to our team.
They shared how their service plays a vital role in supporting older people to receive the right care, in the right place, at the right time — whether that means avoiding unnecessary hospital admissions or supporting safe, supported transitions from hospital back into the comfort of their care home.
Their visit sparked an important conversation on how we can strengthen our processes, partnerships, and communication — ensuring both our team and residents feel supported, informed, and safe, especially during vulnerable transitions.
This session was arranged thanks to the thoughtful outreach of our Business Development Manager, Bridgitte, who reached out to the Hospital at Home Team not just to explain what we do at Carlton Court, but to ask something even more important:
“How can we work better together to support our residents and our care team?”
This kind of forward-thinking, collaborative work is at the heart of delivering truly person-centred, safe, and responsive care — values deeply aligned with the CQC’s expectations and, more importantly, our own.
As a team, we came away with new knowledge, deeper understanding, and fresh ideas to keep improving. We’re proud to keep building these relationships — because when we open the door to collaboration, we open the door to better care.
