Not all Achilles injuries are Achilles at all.

This young academy footballer presents with pain in the Achilles area, and not responding to rehab; with little to no pain on testing, point of care ultrasound visualised a perfectly normal Achilles tendon! Functional testing however, of his Tib post tendon after visualising tenosynovitis on ultrasound reproduced pain and confirmed clinical diagnosis of PTTD.
Custom 3d printed Orthotics, and targeted strengthening rehab progressed with sports physio and he’ll be back playing sooner rather than later.

Often overlooked cause of forefoot pain, metatarsal stress fracture.
Here seen on the 5th metatarsal.
Classic too much too soon, after too little for too long.
Bones need load to be strong, and they need to recover after strenuous exercise just like everything else.

Make sure we are all making a solid diagnosis before we start treatment.
We should be practicing as autonomous practitioners, and as such should question another clinicians diagnosis through the lens of our own expertise and experience.
We all miss things as we’re human, BUT as part of a team we shouldn’t all be missing the same thing.

Another reason to see a foot specialist with a foot problem!
This active lady came in with an existing diagnosis of Morton’s neuroma, supposedly confirmed by NHS ultrasound.
Except little of her history fitted with that diagnosis. The pattern and nature of her symptoms did not fit. And I put a LOT of emphasis on that!
Clinical assessment also prompted more was going on, and following a very fast MRI turn around within 5 days we unfortunately confirmed my suspicion of a Freiburg’s infraction, an avascular necrosis of the 2MTP head. Where the tip of the bone loses its blood supply and starts to die.
The earlier this is treated the better, to minimise bone damage. I expect with treatment this lady will be feeling better soon.

A first for me…
Yesterday I was lucky enough to be doing my nail surgeries in theatre with the full surgical team looking after me….. i mean looking after my patients! 😂
This is how all minor ops will be done now and may be a great option for any patients anxious about such procedures.
