Lymphedema—a common but often overlooked side effect of breast cancer treatment.

Asking the right questions early can be one of your most powerful tools for protecting your long-term health. Here are 5 essential questions to discuss with your care team:

1️⃣ “What is my personal risk of developing lymphedema?”
Your risk depends on the type of surgery or treatment you receive. Factors such as a high BMI, obesity, axillary lymph node dissection (ALND), radiotherapy and chemotherapy can increase the likelihood of lymphedema.

2️⃣ “Will lymph nodes be removed during my surgery—and how many?”
The more lymph nodes removed, the higher is the risk.

3️⃣ “What steps can I take before and after treatment to lower my risk?”
Before and after surgery, ask your care team or Lymphatic drainage specialist about a personalized prevention plan. This may include bioimpedance measurements to track changes in lymphatic flow and detect early signs of lymphedema, along with physical therapy, lymphatic drainage, oncology scar healing, gentle exercises, and healthy lifestyle. Staying proactive and monitoring your health closely can make a big difference in lowering your risk.

4️⃣ “What early warning signs should I watch for?”

Lipedema is more than just “hard-to-lose fat.” It’s a chronic disorder with a strong genetic component that primarily affects women.

While genetic testing is not yet a standard diagnostic tool, research is rapidly evolving to understand how specific gene polymorphisms (like PPARG, Leptin, and Estrogen Receptor genes) play a significant role in lipedema’s development. Identifying risk factors could:

• Improve Early Detection in families with a history.

• Guide Personalized Therapies for metabolic, hormonal, or inflammatory pathways.

• Help Differentiate lipedema from obesity or lymphedema.

Key Genes:

PPARG Regulates fat storage and adipogenesis. Variants here may influence the unique fat distribution and inflammation seen in lipedema.

LEP / LEPR (Leptin) Governs hunger and metabolic function. Polymorphisms may contribute to metabolic dysfunction and adipose tissue expansion.

ESR1, ESR2 & CYP19A1 (Aromatase) Linked to fat distribution, inflammation, and local estrogen production in fat tissue. This may explain why lipedema predominantly affects women.
