April is Sjögren’s Syndrome Awareness Month.

Sjögren’s syndrome is a chronic autoimmune condition where the body’s immune system mistakenly attacks its own moisture-producing glands. This mainly affects the eyes and mouth but it can also impact other parts of the body.

Common symptoms include:

• Persistent Dry Eyes (gritty, burning or itchy feeling)
• Dry mouth (difficulty swallowing, speaking or tasting)
• Fatigue or ongoing tiredness
• Joint pain or stiffness
• Swollen salivary glands (especially around the jaw)
• Dry skin or rashes
• Brain fog or difficulty concentrating

The Schirmer test is a reliable way to assess aqueous deficiency, which occurs when the eyes do not produce enough of the watery component of tears.

During the test, a small strip of paper is gently placed between the lower eyelid and the eye to measure tear production. It is quick, simple, and generally comfortable, taking about five minutes to complete.

Normal results can help rule out Dry Eye caused by reduced tear production, making this test a valuable diagnostic tool for identifying the underlying cause and guiding accurate treatment.

Basic Artificial Tears: Adds quick moisture for mild dryness, screen fatigue or tired eyes

Preservative-Free Drops: Same hydration as artificial tears but gentler for sensitive eyes or frequent use

What an incredible experience at the All India Ophthalmological Society 2026 Convention in Jaipur! It was truly inspiring to be among some of the world’s most respected ophthalmologists at such a massive gathering — with over 10,000 registrants and an outstanding range of scientific sessions and discussions.

We were deeply honoured to be invited by AIOC to speak on the growing challenges of paediatric dry eye, an area that urgently needs more awareness and action. A special thanks to Professor Ken Nischal for the kind invitation to be part of the WSPOS symposium — a privilege to contribute alongside global experts.

Spring can be difficult for people with Dry Eye Disease. Higher pollen levels can trigger Allergic Conjunctivitis, which may make dry eye symptoms worse. These simple steps can help protect your eyes during pollen season.

1. Wear sunglasses outdoors
Sunglasses, especially wraparound styles, help protect your eyes from pollen, wind and dust.

2. Rinse your eyes and eyelids
After being outside, gently wash your eyelids or rinse your eyes to remove pollen and other irritants.

3. Use lubricating eye drops
Preservative-free artificial tears can help keep your eyes comfortable and support the tear film.

4. Avoid rubbing your eyes
Rubbing your eyes can release more histamine and make itching and inflammation worse.

5. Reduce pollen exposure
Keep windows closed on high pollen days and shower or change clothes after spending time outdoors.

Top 5 mistakes making your Dry Eye worse!

Think your Dry Eye is “just one of those things”? You might be accidentally making it worse every single day…

1️⃣ Not blinking enough on screens

When using phones or laptops, your blink rate can drop by up to 50%. You also tend to blink incompletely, which stops the protective oil layer of your tears from spreading properly, leading to faster tear evaporation and irritation.

2️⃣ Using the wrong eye drops

Many redness relief drops contain ingredients that temporarily shrink blood vessels. They may make your eyes look whiter short term, but with regular use they can cause rebound redness, irritation and worsen underlying Dry Eye.

3️⃣ Over wearing contact lenses

Every contact lens reduces the amount of oxygen reaching the front surface of your eye. This disrupts the tear film, increases inflammation and can leave your eyes feeling dry, gritty and sore.

4️⃣ Skipping eyelid hygiene

Your eyelids contain oil glands (meibomian glands) that prevent your tears from evaporating too quickly. If these become blocked due to debris, makeup or inflammation, your tear film becomes unstable, causing evaporative Dry Eye.

5️⃣ Dehydration

Not drinking enough water, especially alongside caffeine or alcohol, can reduce the quantity and quality of your tears, making symptoms like burning, stinging, and blurred vision more noticeable.

Here are some simple fixes that will help:

-Follow the 20 20 20 blink rule
-Use preservative free artificial tears
- being sure to remove all make up daily and being careful you don’t put eye creams too close to the eyelid margins
-Reducing your contact lens wearing time to give your eyes a break in the evenings or even a whole day a week
-Stay hydrated throughout the day

A significant number of patients presenting for cataract surgery have undiagnosed Dry Eye disease and meibomian gland dysfunction (MGD). An unstable tear film can negatively affect biometry and corneal topography, increasing the risk of refractive surprises, particularly when using toric and multifocal intraocular lenses. Poor tear film quality has been associated with inaccurate measurements, reduced visual quality, and lower patient satisfaction, even when cataract surgery is technically successful.

MGD and untreated Dry Eye have the potential to compromise pre-operative measurements, highlighting the importance of incorporating thorough meibomian gland evaluation and Dry Eye screening as a standard and essential component of the cataract surgery assessment.

If you experience Dry Eye symptoms such as irritation, watering, or flare-ups, these tips may help improve comfort:

Hydration:

Staying well hydrated supports healthy tear production. Drink water regularly and limit excessive caffeine or alcohol, which can worsen dryness.

Screen Use:

We blink less when using screens, which can worsen Dry Eye. Reduce screen time where possible or follow the 20-20-20 rule — every 20 minutes, look 20 feet away for 20 seconds.

Lid Hygiene:

Dust, wind, and debris can trigger flare-ups. Good eyelid hygiene and eye protection can help reduce irritation.

Dry Eye affects everyone differently. Identifying your triggers is key to managing symptoms.
