Tear trough filler is one of the most requested treatments I see, and also one of the most commonly done on the wrong candidate.

The under-eye area is one of the most unforgiving places to place filler, and getting a natural, long-lasting result depends entirely on the quality of the skin that’s already there. Thin, lax, or compromised skin under the eyes won’t hold filler well, and placing it without addressing that first is one of the most common reasons people end up disappointed with their result.

This is why assessment matters so much before any tear trough treatment. A simple pinch test tells us a great deal about the skin’s integrity and how well it’s likely to respond. If the skin is slow to snap back, that’s a sign that skin quality needs to be addressed first, and in that case, polynucleotides or other skin quality treatments will give a far better foundation to work from before we consider filler.

Tear trough filler tends to work best in younger patients, typically under 40, where the skin still has good elasticity and support. For everyone else, the starting point is almost always improving the skin itself.

Patients come to us having spent money on lasers, peels and IPL elsewhere, seen results, then watched it all return a few months later. Frustrated and convinced that nothing will work for them.

The most important step in any pigmentation plan is the one most clinics skip entirely. Suppressing the melanocytes before any heat or resurfacing treatment is introduced. Without that foundation, you are treating the surface without addressing what is driving it underneath.

Pillow face usually happens when filler is placed when we treat the symptom, not the cause. This is one of the most common issues I see when patients have had previous filler elsewhere.

They’ve often had product placed centrally, around the nasolabial folds or marionette lines, because those are the areas they’re concerned about. But if the real issue is loss of support higher up, adding filler directly into those folds can make the face look heavier, puffier and less natural.

That’s why assessment really matters. The midface and lateral face often need support first. When we restore structure higher up, it can help lift and rebalance the tissues so the central face softens without needing to overload it.

Good filler isn’t about filling every crease - it’s about understanding how the face is ageing, where support has been lost, and what will give the cleanest, most natural result long term.
