Tamoxifen Tuesday: Managing changes to periods🌺

Irregular periods or missed periods can happen because tamoxifen blocks estrogen receptors in reproductive tissues💊🚫

A missed period doesn’t automatically mean menopause❌

If your period stops it may return after finishing tamoxifen for some people⏰

But if you’re on tamoxifen for years (sometimes up to 10 years) periods may not return because you may enter menopause naturally over that time💁‍♀️

Metastatic Mondays: Using blood tests to change treatment before scans show progression🩸

‼️ This post is about metastatic HR+ breast cancer in people taking hormone therapy (camizestrant vs standard endocrine therapy) alongside a CDK4/6 inhibitor‼️

In the SERENA-6 trial, researchers looked at whether monitoring circulating tumour DNA (ctDNA) — tumour DNA found in the blood — could help guide treatment decisions🧬

They specifically monitored for an emerging ESR1 mutation, which can be a sign that the cancer is starting to develop resistance to the current hormone therapy👩‍⚕️

If an ESR1 mutation appeared in the blood, people switched their hormone therapy earlier, while continuing on the same CDK4/6 inhibitor💊🔄

Why this matters: This supports a future approach where treatment changes can be guided by a blood test, meaning someone may be able to switch therapy before scans show progression🙏

⚠️Important note: Camizestrant is still investigational and isn’t standard care yet⚠️

Molecular subtypes are based on specific characteristics of cancer cells🧫⬇️

🔸Hormone receptor-positive (HR+) - cancer growth influenced by estrogen and progesterone hormones

🔸HER2-positive (HER2+) - HER2 protein is expressed in high amounts leading to abnormal cell division and growth

🔸HER2-low - low amounts of the HER2 protein (new evidence suggests up to 55% of all breast cancers may be HER2-low)

Here’s some tips⬇️✨

🔸Reduce salty or processed foods (salt can make you retain more fluid)🧂

🔸Try elevating your feet/ankles when resting (prop them up on a pillow)🦶

🔸Avoid long periods of standing or sitting - gentle movement can help circulation🚶‍♀️

🔸Wear loose fitting clothing if swelling makes things feel tight or uncomfortable👚

🔸Elevate your head slightly at night if you wake with puffy eyes🛌

🔸Ask your care team if compression socks may help🧦

🔸Speak to your care team if swelling is painful, severe, or getting worse👩‍⚕️

Metastatic Mondays: Updates on Datroway for metastatic TNBC💜

The TROPION-Breast02 trial tested Datroway (deruxtecan / Dato-DXd) as a first-line treatment for TNBC in patients who aren’t eligible for immunotherapy🥼🧑‍⚕️ It was compared against standard chemotherapy💊

The results found⬇️

🔸Datroway delayed disease progression (PFS): 10.8 months vs 5.6 months on chemo⏰

🔸Datroway improved overall survival (OS): 23.7 months vs 18.7 months on chemo📈

Datroway could become a new chemo-alternative for some people with metastatic TNBC, once approved💊

Breast cancer is often described by where the cancer starts (ducts vs lobules) and whether it has invaded nearby breast tissue (in situ/non-invasive vs invasive 💜

In situ (non-invasive) means the cells are still contained in their original site and haven’t spread into surrounding breast tissue:

🔹 DCIS - abnormal cells in the milk ducts (often called stage 0). It isn’t invasive, but it can become invasive over time if untreated.

🔹 LCIS - it isn’t usually considered breast cancer. It is benign (non-harmful) but it may increase the risk of developing breast cancer in the future

Invasive (also known as no special type or NST) means the cells have spread into nearby breast tissue:

🔹 IDC - the most common invasive type (starts in ducts) and accounts for around 80% of invasive breast cancer cases

🔹 ILC starts in lobules and accounts for around 10% of invasive breast cancer cases.
