NINE TENDINOPATHY FACTS

1) Tendinopathy does not improve with rest – the pain may settle but returning to activity is often painful again because rest does nothing to increase the tolerance of the tendon to load.
2) Although there are some inflammatory biochemical and cells involved in tendinopathy, it is not considered to be a classic inflammatory response. Anti inflammatories may help if you have very high pain levels but it is unclear what effect they have on the actual cells and pathology.
3) Tendinopathy can be caused by many different risk factors. The main factor is a sudden change in certain activities – these activities include 1) those that require the tendon to store energy (i.e. walking, running, jumping), and 2) loads that compress the tendon. Some people are predisposed because of biomechanics (e.g. poor muscle capacity or endurance) or systemic factors (e.g. age, menopause, elevated cholesterol, increased susceptibility to pain, etc). Predisposed people may develop tendon pain with even subtle changes in their activity.
4) Exercise is the most evidence based treatment for tendinopathy – tendons need to be loaded progressively so that they can develop greater tolerance to the loads that an individual needs to endure in their day-to-day life. In a vast majority of cases (but not all) tendinopathy will not improve without this vital load stimulus.
5) Modifying load is important in settling tendon pain. This often involves reducing (at least in the short-term) abusive tendon load that involves energy storage and compression.
6) Pathology on imaging is NOT equal to pain – pathology is common in people without pain. Also, if you have been told you have ‘severe pathology’ or even ‘tears’ this DOES NOT necessarily mean you will not get better or have a poorer outcome. Further, we know that even with the best intentioned treatment (exercise, injections, etc) the pathology is not likely to reverse in most cases. Therefore, most treatments are targeted towards improving pain and function, rather than tissue healing, although this still is a consideration.
7) Tendinopathy rarely improves long term with only passive treatments such as massage, therapeutic ultrasound, injections, shock-wave therapy etc. Exercise is often the vital ingredient and passive treatments are adjuncts. Multiple injections in particular should be avoided, as this is often associated with a poorer outcome.
8) Exercise needs to be individualised. This is based on the individual’s pain and function presentation. There should be progressive increase in load to enable restoration of goal function whilst respecting pain.
9) Tendinopathy responds very slowly to exercise. You need to have patience, ensure that exercise is correct and progressed appropriately, and try and resist the common temptation to accept ‘short cuts’ like injections and surgery. There are often no short cuts.

The role of physiotherapy in the management of lower back

Lower back pain is one of the leading causes of physical disability, especially at the workplace. Research indicates that almost 80% of the population is likely to suffer from lower back pain in their lifetime.

Physiotherapy and Low Back Pain
Physiotherapy is one of the most widely used forms of treatment adopted for gaining relief from low back pain. It is used in both modes, as a single line of treatment as well as in combination with other treatments such as massage, heat, traction, ultrasound or short wave diathermy.

The human back is basically a highly complex system of series of interlocking elements including the vertebrae, discs, facet joints, ligaments and muscles. Owing to such a complex structure, an episode of back pain needs a strong physiotherapy-based rehabilitation program, once the basic medication course has been undertaken.

Key Aspects
In this section, I have briefly listed the key aspects of the role of physiotherapy in management of lower back pain.

Advice and early activity – There is significant evidence to prove that encouraging early movement in case of lower back pain is one of the most significant aspects of treatment in this condition.
Mobilization or Manipulative physiotherapy – This aspect concentrates on promoting mobilization of the specific affected area. The approach of manipulative physiotherapy is used to target the specific point of pain for the purpose.
Specific stabilization exercises – In this aspect of physiotherapy, stress is laid on improving the strength and stability of the muscles which have been weakened due to the lower back pain.
General exercises and stretches – A series of properly structured exercises and stretches are usually carried out in context of the patient’s individual condition and cause of the lower back pain.
Ergonomic advice – Since work-related hazards account for more than 65% of lower back problems, physiotherapists also concentrate on providing accurate ergonomic device, guiding the patient on using the appropriate infrastructure at work to avoid and cure lower back pain.
Postural guidelines – This aspect of physiotherapy focuses on guiding the patient about the correct postural habits and ways to maintain accurate posture to avoid lower back pain.

With September now upon us and summer coming to a close, many of us are returning to our work. Additionally, many children and young adults will be starting a new academic year; whether that be at school, college and University for many! Here are some postural tips and advice for the work environment.
Lower back pain has become a major issue
As many of our roles will require us being sat at a desk for prolonged periods of time. Lower back pain and subsequent disability have become a prevalent issue. It is now estimated that 80% of individuals will experience an episode of lower back pain on one occasion in their lives. Lower back pain is the highest cause of disability worldwide so this is a topic which really requires better understanding.
For that reason, we have devised some quick tips we can all aim to use and incorporate into our daily routines. These can be used whilst in our work environment, to help prevent the development of symptoms affecting the lower back. Nonetheless, neck and upper back pain also remain prevalent whilst we work and therefore these tips should also be used to address these issues too!
Quick postural tips and advice for the work environment
Regular Breaks: Ensure you have frequent breaks throughout the day. This can be simply standing up whilst at your desk at regular intervals throughout the day or making a drink or heading to the bathroom. These can help decrease the risk of you developing back and neck pain.
Range of Movement Exercises: These can be performed whilst at your desk such as: rolling and active movement of the of the neck and head, as well as the shoulder blades, aiding to facilitate ongoing and regular movement of your joints and muscles throughout the day.
Ergonomic and Desk Based Assessment: An ergonomic assessment at work will assess your posture and keep it to an optimum throughout the day. Following the completion of a desk assessment, you will understand whether certain technicalities, such as altering the height of your chair, will help in addressing and preventing ongoing symptoms.
Remain Physically Active: Remaining physically active and interested in a sporting activity, will always aid in reducing the impact of many musculoskeletal conditions. Maintaining and enhancing bodily strength, endurance, stability and flexibility throughout your spine and trunk, can significantly lower the risk of developing problems whilst sat at work.

Top 10 tips on managing running injuries
TIP 8 Plan a gradual return to running – so many people expect a rapid return to where they left off as soon as symptoms have settled. Unfortunately the body needs time to adapt to running again and any long break from running requires a gradual return. One way to do this is to plan it like you might a race. Set your goal as your normal weekly mileage and plan your running building up to this. Start by finding where your new level is – the maximum distance you can run without causing pain (during or after). Build up from there and but don’t increase your weekly mileage by more than 10% per week.

Rick Yates Physiotherapist

Rick’s interest in sport lead him to complete a BSc (Hons) degree in Physiotherapy in 2002. He is a member of the Healthcare Professions Council, the Chartered Society of Physiotherapy and the Acupuncture Association of Physiotherapy.

Since then, he has worked in acute NHS trusts, private clinics and the private sector and, throughout this time, has specialised in the treatment and rehabilitation of musculoskeletal injuries and conditions. Over the years, he has progressed to become a clinical specialist in sports injuries and musculoskeletal physiotherapy treating all manner of conditions such as muscle and ligament injuries, back and neck pain, sciatica, tendinopathies, biomechanically-related dysfunctions, work-related disorders, RSI injuries, whiplash and post surgical rehabilitation. Rick also regularly undertakes industry and private ergonomic DSE assessments.

He is a highly passionate and dedicated physiotherapist with 15 years experience in treating muscle and joint injuries, longstanding pain and sports injuries. Paramount to his success is that he doesn’t just treat the symptoms but also identifies and addresses the cause of the problem to prevent future ongoing issues. In order to do this, he uses a holistic approach, looking at the entire biomechanical chain and the impact previous injuries may have had on the current symptoms. Treatment techniques that Rick specialises in include manual therapy, exercise prescription, soft tissue and trigger point release, taping, acupuncture and cupping.
