Role Overview
As a Care Coordinator, you will play a pivotal role in managing and organising care services for our clients. You will be responsible for scheduling care visits, liaising with clients and care staff, and ensuring that all care packages are delivered efficiently and to the highest standard. Your ability to multitask, communicate effectively, and adapt to changing situations will be crucial in this role.
Key Responsibilities
Coordinate and schedule care visits, ensuring optimal matching of carers to clients based on skills, experience, and client preferences.
Maintain accurate and up-to-date records of care schedules, client information, and staff availability using our care management system.
Act as the primary point of contact for clients and their families, addressing inquiries and resolving any concerns promptly and professionally.
Collaborate with the recruitment team to ensure adequate staffing levels and participate in the onboarding process of new care staff.
Monitor the quality of care provided, conducting regular reviews and assessments to ensure compliance with care plans and regulatory standards.
Work closely with the Registered Manager to implement policies, procedures, and continuous improvement initiatives.
Participate in the on-call rota to provide support outside of regular working hours as needed.
Essential Requirements
Previous experience in a care coordination or scheduling role within the health and social care sector.
Strong organisational skills with the ability to manage multiple tasks and priorities effectively.
Excellent communication and interpersonal skills, with a customer-focused approach.
Proficiency in using care management software and Microsoft Office applications.
Knowledge of CQC regulations and standards.
Ability to work independently and as part of a team in a fast-paced environment.
Full UK driving licence and access to a vehicle.
Desirable Attributes
NVQ Level 3 in Health and Social Care or equivalent qualification.
Experience in domiciliary care settings.
Familiarity with electronic call monitoring systems.
