Summary of responsibilities and duties:

Personal Care:
To carry out all personal care tasks – excluding those defined as nursing duties – to assist and support a person to live independently in the community as per their individual assessed need.

· Assisting in and out of bed, dressing/undressing

· Assisting with personal washing/bathing/showering/hair care

· Assisting to use toilet/commode

· Emptying commodes and catheter bags. Changing catheter bags as necessary; disposing of incontinence pads

· Cleaning and changing after soiling and wetting

· Assisting with eating - where necessary

· Prompting with medication – subject to medication policy and procedures

· To undertake any other health related tasks which are listed in the care plan and are not classified as nursing tasks.

· To encourage and assist the service user to use any aids provided – as per the instructions of the relevant professional

· To report any changes in circumstance to the office immediately

Social Care Duties:

· To establish a relationship with a service user, giving support and providing a link with the community

· Making telephone calls to doctors, emergency services or to make appointments and, where necessary, accompany the service user to the hospital/doctor

· Reading and writing letter and completing forms

· Motivating and assisting the service user to regain lost skills and promote their independence

· Deal with callers to a service users home

Domestic Duties:

· Cleaning and tidying the service user’s home

· Making and changing beds

· Preparing drinks and cooking meals

· Laundering and ironing clothes

· Preparing soiled laundry for collection by laundry service when required

· Shopping, pension collection, bill payment (duties involving handling money should be cleared with the office first and carefully recorded)
