WE ARE HIRING A CARE COORDINATOR
We are currently seeking an experienced Care Co-ordinator to join our team. The role of the Care Coordinator is to support the delivery of the highest quality care support services to clients living within their own homes including the assessment and review of individual services and the practical supervision, instruction and support of care workers. You must be a driver with your own vehicle.
Duties include:
- To assist the Registered Care Manager in the delivery of care services ensuring the service operates to specified standards.
- To establish and maintain effective working relationships with Adult Social Care Teams, Continuing Healthcare Teams, Mental Health Teams and Health Professionals.
- To establish and maintain effective working relationships with clients and their families/advocates/informal carers and respond to any concerns or queries raised in a timely manner.
- To carry out initial visit assessments with any client new to the service and complete risk assessments and care plan documentation as required.
- To ensure all Task Plans, Risk Assessments and Care Plans are kept up to date and meet clients’ needs and respond to changes effectively within agreed timescales.
- To directly supervise, instruct and support care workers in their day to day duties.
- To ensure appropriate responses are made to the clients’ suggestions and complaints about the service provided and are reported to the Registered Care Manager/Director within the response times.
Skills:
The ideal candidate should possess the following skills:
- Minimum 2 years’ experience of providing person centred support to adults in a community setting
- Strong organisational skills with the ability to multitask effectively
-Experience of planning and organising Home Support staff rotas via the use of rostering software to maximise resources (a knowledge of StaffPlan rostering and Birdie Care Management systems would be an advantage)
- Ability to work well under pressure and adapt to changing priorities
-Ability to communicate clearly and build positive working relationships with colleagues, clients and their families/advocates, health and social care professionals
-Ability to maintain clear and accurate records and follow statutory reporting procedures including safeguarding.

Pay: £25,400.00-£26,000.00 per year
