π’ Important Update β New Way to Access Care
From Monday all appointment and admin requests will go through our online Patient Triage form.
You can access it:

By calling us β if you canβt complete the form, our reception team will help you complete the form.
Forms are open and available from 8am through to 6pm Monday to Friday.
β° Same-day urgent requests (including home visits) must be submitted by 11 am.
After this cut-off you can still request routine/non-urgent appointments or submit admin queries.
Every request is reviewed by a GP/Navigation team the same day. You may be:
π©ββοΈ Booked with the GP
π Referred to another clinician such as pharmacist, physio or mental health worker
π² Sent self-care advice
If we reach safe capacity for the day or you submit an urgent request after the cut-off, you may be directed to NHS 111 or a local Walk-In Centre.
π Medication requests should continue via the NHS App.
