Mission CESR Episode 2

Evidences needed to be collected in the UK whilst working in the NHS

This post is meant for all middle grade doctors and locum consultant doctors (in the NHS) who are planning to apply for CESR in their respective fields.

All the below mentioned evidences need to be attested by a senior colleague except CPD Certificates.
All Patients data like NHS numbers, Names etc should be anonymised.
Any junior colleagues personal data whom you have assessed also should be anonymised.

1. Contracts , Job Plans , Duty Rotas
2. Appraisals
3. CPD Diary & Certificates
4. All relevant course Certificates like ALS,EPALS, Educational Supervisor Course Certificate, Any Management related course Certificates etc.
5. EPortfolio Report
6. Multi Source Feedback (Colleagues and Patients)
7. QIP/Audit Certificates , Letters and Power Points(Full cycles are essential)
8. Teaching evidence like Power Points and Feedbacks
9. Procedure Log Books, Patient Lists and Cumulative Data of Inpatient and Clinic.
10. Abnormal Investigations list ordered in your name (Biochemistry , Pathology and Radiology)
11. GP letters, Inter Departmental Letters
12. MDT minutes with your name mentioned
13. M/M , Clinical Governance Meeting, Consultant meeting, Departmental meeting minutes with your name mentioned
14. Reference forms, EPortfolio assessor data (For consultants)
15. Colleague letters stating your roles and competency and compliments if any.
16. Medical Reports (Coroner's Report, Complaint response reports,DVLA reports etc)
17. Any Discretionary points or letters.
18. Head of Service Report
19. Publications, Research and Posters.
20. Vaccination and Health records from occupational health.
21. Any other letter stating your senior role and competency would be useful

There's no hard and fast evidence of how many of each or if more are required and the list is never ending and quite exhausting.
The above mentioned is a brief summary of the evidences required.

MISSION CESR EPISODE 1

CESR Evidences from Home Country for General Internal Medicine or related Specialties

Hello everyone, this post is meant for all doctors in UK who are in middle grade position and above in Medicine and related Specialties and for doctors from abroad who are looking to join as Middle grade or above who wants to pursue CESR pathway to become Substantive Consultants in NHS.
I will briefly list out all the evidences required from your home country which will form essential Evidence for your CESR Application.

Please note that evidences from last 5 years of application are acceptable and not beyond that.

1. Specialty Degree ( MD, DNB, DM , FCPS etc) Copy which is Notarized.
2. Specialist Registration ( MCI, State council registration etc) Copy which is Notarized.
3. Additional Qualifications Copies like fellowships or diplomas notarized.
4. PG Log Book anonymised and attested by someone in your Post Graduation Institution.(Only procedures section).
5. Postgraduate Curriculum of your Specialty.
5. Appointment letters, Contracts, Experience letters and Job plans from home country since the last 5 years.
6.Duty Rotas example and Teaching Timetables examples since the last 5 years. (Including power point presentations)
7. Patient lists - Inpatient and Outpatient, Cumulative datas, Procedures all attested by relevant supervisory colleagues.
8. CME CPD Certificates since 5 years.
9. Awards and Honors.
10. Patient and Student Feedbacks
11. Patient Letters or GP letters.
12. Any document telling your senior role in patient management or quality improvement such as Mortality Morbidity, Clinical Audit Letters and Certificates, Clinical Governance meetings minutes etc.
13. Letters of recommendation and colleague letters stating your role and competencies.
14. Compliments and Complaint Resolution letters.
15. Case evaluation or discussions or extensive medical reports about the cases.

The list is never ending and comprehensive and this is just an example and can include many more.
Most of the documents should be attested by relevant supervisory colleagues.

Tier 2 Visa Changes

Effective from August, Tier 2 Visa for doctors working in the NHS is called Health and Care Visa.

It's easier,quicker & cheaper.

The eligibility criteria remains the same and application process remains the same.

The salient differences are
1. It's now GBP 232 per head for a visa upto 3 years.
2. It's now GBP 464 per head for a visa more than 3 years. (Same for the dependents)
3. No immigration Health Surcharge as previously.(even for dependents)
4. Quicker processing upto 3 months.

CESR CP

A good news to all MD / DM degree holders in India , and surrounding countries (Ones who finished Specialty Training for 3 years and hold a Post Graduate Degree in the same and have some experience or did a Fellowship or Super specialization in a Subspecialty like cardiology, gastroenterology, nephrology, nuerology, ICU) etc

You can join Specialty Training via a route called CESR CP where, you can join a specific department like Cardiology etc in a ST3 role (Specialty Training Year 3) , finish around 4 years of British Training in that respective branch and be a permanent Consultant in the NHS.

Until this year, the doctors who went through the above mentioned route used to be awarded CESR CP, But from this year, doctors who trained in the UK via this route will be awarded CCT (Certificate of Completion of Training) which is the highest achievable clinical degree in the UK.

This post is about ways to become a substantive or a permanent Consultant in the UK.

This post is for doctors who already completed their Specialty Training in their home countries and then finishing their MRCP and relocating to the UK.

There are 2 ways of becoming a Substantive Consultant in NHS-
1. CESR
2. CESR CP (Now being changed , CCT is being awarded)

Today we will discuss about CESR

CESR
Certificate Eligibility for Specialist Registration

Needs a truckload of evidence to be submitted to the GMC proving that you meet the requirements to become a full fledged Consultant in your Specialty from home country and present UK work place.
Need to apply for it after minimum of 1 year or working in the NHS. Usually takes around 2 to 3 years minimum.

What to collect in your home country at workplace or training place before coming to the UK ??

Briefly I am adding a summarized list, might definitely need more

1. LOGBOOK - All postgraduate courses in India have a logbook, please preserve that. It's very important to prove your competency.
2. Postgraduate Curriculum for the course from your University / Your postgraduate degrees and registrations notarized.
3. LOGBOOK from workplace if any
4. All job plans,duty rotas, teaching schedules if you were involved, case reports etc.
5. All joining letters, employment contracts, experience letters , recommendation letters from employers, colleagues and seniors etc.
6. Cumulative data of your work, IP data, OP data ,Surgeries conducted etc
7. CME certificates , Conference certificates, Workshop certificates, Talks given certificates etc
8. Publications, Paper presentations abstracts, Poster abstracts and posters etc.
9. Audits if you were involved.
10. Thank you letters and feedbacks from patients , colleagues and students if any.

And any sort of evidence from your previous workplace from your home country proving you competency and role .

The rest of the evidence needs to collected from NHS once you start working in the NHS.

What to do after landing in the UK??

1. Collect BRP (Biometric Residence Permit)

When you apply for Tier 2 visa , once accepted you'll be given a Visa Vignette on your passport which is most commonly valid for 1 month.
Please note this is not your Visa.
This is just the permission to enter UK.
Once you land , you'll have 7 to 10 days within which you need to collect your BRP in the nearest post office which you will mention during visa application.
Your national insurance number will be printed behind your BRP which is now your visa.

2. Take a pay as you go UK SIM Card

There are multiple offers or multiple companies which you can select and buy.
Eg 3 Mobile, Vodafone, EE , Lyca , Lebara

3. If you already have an accommodation set up , collect your tenancy agreement from your landlord, without which you'll not be given any basic thing like a bank account, post paid sim contract etc.

4. Collect a letter for bank account opening from your Hospital HR as its a mandatory thing for account opening .
You'll usually need your BRP, Tennacy Agreement & Hospital letter for account opening.
HR usually asks which bank do you want a letter for.

Selection of bank is a personal choice, there are multiple good banks like Halifax, Barclays, Nationwide, Llyod, HSBC etc
Any one should be good.

Tier 2 Visa Processing

You first need to register in the below mentioned site and complete the self explanatory application and pay your visa and IHS (Immigration Health Surcharge) and book a slot in the local VFS office for photograph and biometrics.
You can put the hospital address in the UK address if you don't have any confirmed address.
You can either submit documents online to the VFS Portal or in person if you select that option.

Documents Required to be Uploaded in the VFS PORTAL

1. Certificate of sponsorship (Issued by the NHS trust after the successful interview and acceptance as a proof of employment which indicates your period of contract, salary, maintenance etc)
2. Police Clearance Certificate (Can be obtained from Passport office, Everything is clearly mentioned in the official website. )
3. TB Clearance Certificate ( From specific countries, valid centers available on the mentioned website).
4. English Test Result.
5. GMC ID Check Invitation Mail Screenshot if applicable.
6. Proof of Savings (if your certificate of sponsorship doesn't have Yes ticked on maintainence. You should atleast have £945 in your account since 90 days .)
7. Address proof in the UK if you have been granted one or you can leave it empty.

Some doctors prefer to add a cover letter explaining the above for a smoother process but it's not mandatory as personally I have not added one.

Please note that all doctor Specialties are now in the shortage occupation list so the visa fee is lower than others.
Please note that IHS fee is mandatory as it covers your health care whilst your stay in the UK and without paying it your application will not be move ahead.

The cost of Tier 2 (General) shortage occupation where a certificate of sponsorship has been issued for a period of three years or less is £464.
The IHS fee is £400 for 1 year Certificate of sponsorship for Tier 2 shortage occupation list visa.
VFS charges a fee varying depending on the services you avail for biometrics.

You need to collect your BIOMETRIC RESIDENCE PERMIT (BRP) after you land as that is your valid visa for your stay. You have to give address of your nearest post office in the UK for collection. BRP arrives at your collection point approximately a week or less from your start date on your visa vignette which is displayed on your passport.

GMC Registration

Prerequisites for GMC Registration

1. EPIC ID
2. CERTIFICATE of Good Standing from all the medical councils you have been registered with since 5 years.
3. MBBS degree
4. Internship Certificate
5. Recognized Post Graduate Qualification (MRCP etc)
6. English Assessment Pass Score (IELTS OR OET)
7. Valid Passport

Please note that while opting for registration via recognized Post graduate qualification route you should not select the others like CCT etc.

Please note that while filling the form,make sure you type all the individual postings you attended during your internship period which is pre graduate in individual columns rather than everything in one.

Please note that you need to fill your last 5 years of clinical and non clinical experience with dates.
Maximum of 28 days of non working is accepted and not more . Make sure you have all the experience letters ready if at all they ask.

Certificate of Good Standing is valid for only 3 months from issue so please apply for registration within that time frame.

EPIC VERIFICATION

EPIC verification is one the most important Prerequisites for GMC Registration

3 things need to be verified by them

MBBS degree
Internship Certificate
Recognized Post Graduate Qualification

Visit https://www.ecfmgepic.org/index.html

Register yourself first.
There's going to be a identification check via Skype from a notary where you need to show your passport online on we**am and digitally sign it. Everything is self explanatory.
Then you'll be given an EPIC ID.
After that you need to upload your MBBS and Internship certificate as mentioned above along with details of your medical college.
EPIC will send those Certificates to your medical college for verification, if your medical college has online verification portal , they will electronically verify and send back to EPIC or if not they will have to manually courier them to EPIC so please find out in your respective medical colleges about their facilities.
For your recognized Post graduate qualification, verification is done electronically.

Please note that this entire process is a very lengthy one and might take more than a month also so please make sure this is started much earlier.

Please note this post is for doctors applying via recognized Post graduate qualification route and not PLAB route.

Your EPIC ID needs to be submitted to GMC once all verifications are complete and when you start filling your GMC registration form.
