Medication practitioner,community-based

ACS Care Services Ltd are excited to offer a new role to assess and support people in their own homes with their medication. This will include assessing, planning, collecting, delivering and administering medication in time critical patients in their own homes across Milton Keynes.

The service will be working together with central and Northwest London NHS foundation trust and supporting district nurses with their caseload.

Hours 35 hours per week rotational split shift between 7 am and 10 pm on a 3.5 day basis.

Salary 26,390+40 per mile

Start date 7th of November 2022

Contract, six month fixed term contract may be extended.

You will need to be a driver with a permanent use of your own vehicle. Your vehicle insurance will need to include business insurance as you’ll be travelling around the community.

Disclosure information will be required for a DBS application.

End of Life(EOL) interim Caregiver

A new and exciting opportunity presents job rolls for E0L caregivers.

ACS Care Services Our commission to provide immediate end of life care to patients in their own homes. The aim of the service is to provide immediate support for EOL patients as identified by the District Nursing service To meet the individuals needs in the gap ahead of a CHC package being fast tracked to a continuing healthcare provider. The care package will take place within four hours of the referral.

Typical immediate EOL patient needs include.:
Personal care/washing
Mouth care
Pressure area care
Medication administration
Support with foods and fluids
And any other tasks to meet the patient’s individual needs.

Patience will require multiple visits each day and all visits will be with two members of staff and this will be delivered in the patient’s own homes across Milton Keynes.

35 hours per week,
Rotational shifts, covering Monday-Sunday 7am-2pm, 2pm- 9pm

Salary £26,390+ 40p per mile

Start date 7th November 2022

ACS Care services are looking for Care support workers, we are looking for caring individuals that are flexible, organised and dedicated. We have various shifts, part time, nights and weekends, experience a bonus but not required, paid training provided. Please note; ACS are not currently requiring male care workers due to service user preference.
