Meeting this client and learning about her concerns with a previous lip augmentation showed we needed to step in and try and resolve the issue.
Filler can migrate on it's own this is well known but to land deep in the border is most likely the result of a practitioner trying to get depth to the lip and using Russian style linear retrograde threads and fanning techniques but placing the needle too deep whilst depositing the filler.
Hyaluronic acid is drawn to water and being placed near the wet dry border it will definitely settle in there and can stay there, undisturbed for many years.
These little pockets of hyaluronic acid were very stubborn and took a lot of massage and manipulating to help break these pockets down. Sometimes pockets like this may always remain partially visible where the skin has adjusted to the volume over the years and stretched slightly.
Hyaluronidase although it is effective at dissolving HA filler, it can be irritating to the skin, this irritation could be viewed negatively. Yes, occasionaly when dissolving antibiotics are required should the hyaluronidase cause cellulitis. However, in this case, the irritation triggers and accelerates the four stages of wound healing (hemostasis, inflammation, proliferation, and maturation) without causing cellulitis.
It functions by degrading hyaluronic acid (HA), producing low-molecular-weight HA fragments that stimulate the body's repair mechanisms, including increased cellularity, fibroblast proliferation, and rapid angiogenesis.
This means through healing the skin can tighten due to producing more collagen and elastin.
Exactly what was needed here.
