NEW VACANCY - CLIENT LIAISON OFFICER
We are currently looking to recruit a client liaison officer to join our growing office team.
Part time role – 3 days a week 9am-5pm (work pattern to be agreed– weekdays only)
The main duties of the role include:
• Initial assessments
• Setting up care packages
• Risk assessing the home environment
• Creating Care plans that are tailored to each individuals needs
• Completing reviews with Social workers and other health care professionals
• Reviewing the care plans on a three-month basis or more frequently when changes are required
• Office duties that include liaising with our clients, families, health professionals, and care team
• Making referrals to relevant bodies and health professionals
To apply for the role we do require:
• That’s you have excellent IT skills and are able to work with drop box, word, excel, share point and other IT systems
• Full UK driving licence – as you will be visiting individuals in their own homes
• Previous experience in a similar role
• Experience / good understanding with writing care plans
• Experience with assessing packages of care.
Rates of pay to be discussed and agreed depending on experience
