เรื่องการนวด "เพื่อสลายพังผืด" แบบไหนนวดได้ แบบไหนไม่ควรนวด และควรนวดช่วงไหน ระยะไหน จึงจะปลอดภัยกับคนไข้ที่ถูกนวดมากที่สุด และไม่กลับไปอักเสบซ้ำหรือเกิดพังผืดหนาขึ้นมาซ้ำเติมจนหนากว่าเดิม

หมอนวดหลายคน เวลาจับเจอว่ากล้ามเนื้อของคนไข้ค่อนข้างแข็งหรือตึง มักจะบอกว่าอาการของคนไข้ "มีพังผืดอยู่เยอะ" ต้องค่อย ๆ นวดให้พังผืดสลายตัวออกไปและเดียวอาการก็จะค่อย ๆ ดีขึ้น - ขอแนะนำว่า อย่าเพิ่งไปบอกคนไข้แบบนั้นเด็ดขาด จนกว่าจะตรวจและวินิจฉัยอาการโดยละเอียดก่อน

*** หมอนวดบางส่วน เวลาพูดเรื่องของ "พังผืด" ไม่ได้หมายความว่าเขารู้เรื่อง "พังผืด" จริง ๆ ส่วนใหญ่พูดตามที่เรียนมา ว่าถ้ากล้ามเนื้อเกร็งตึงในลักษณะแบบนี้ เกิดจาก "พังผืด" ไปเกาะกล้ามเนื้อ แต่ไม่รู้ว่ากระบวนการเกิดขึ้นมันเกิดได้ยังไง จากสาเหตุอะไรได้บ้าง

กล้ามเนื้อที่เกิดอาการแข็ง หรือเกร็งตึง ที่เกิดจาก "พังผืด" ไปเกาะที่กล้ามเนื้อ แน่นอนว่า "พังผืด" มันไม่ใช่ว่าอยู่ดี ๆ มันจะเกิดขึ้นมาเองได้ง่าย ๆ และในทางการแพทย์มันมี "พังผืด" อยู่ 2 รูปแบบ คือ Fascia และ Fibrosis

1) Fascia (พังผืด) ความหมาย: - Fascia คือ เนื้อเยื่อเกี่ยวพัน (connective tissue) ชนิดหนึ่งที่ห่อหุ้มและเชื่อมโยงโครงสร้างต่าง ๆ ของร่างกาย เช่น กล้ามเนื้อ เส้นเอ็น กระดูก เส้นประสาท และอวัยวะภายใน

*** องค์ประกอบ

- ส่วนใหญ่เป็น collagen fiber และ elastin
- มี matrix (ground substance) และเซลล์ fibroblast

*** หน้าที่หลัก

- รองรับและค้ำจุนโครงสร้าง (support)
- ช่วยกระจายแรงและการเคลื่อนไหว (force transmission)
- ทำหน้าที่เป็นเส้นทางของหลอดเลือดและเส้นประสาท
- เชื่อมโยงเป็น “continuity” ของร่างกาย เช่น deep fascia, superficial fascia, visceral fascia

สถานะทางสรีรวิทยา - Fascia เป็น โครงสร้างปกติ ของร่างกาย ไม่ใช่โรค

2) Fibrosis (พังผืด/การเกิดพังผืด) ความหมาย - Fibrosis คือ ภาวะทางพยาธิสภาพ (pathological condition) หมายถึง การที่ร่างกายมีการสะสมของ collagen และ extracellular matrix มากผิดปกติ อันเป็นผลจากการอักเสบหรือการซ่อมแซมที่ไม่สมดุล

*** กลไกการเกิด

- กระตุ้นโดยการบาดเจ็บ/อักเสบเรื้อรัง fibroblast และ myofibroblast ถูกกระตุ้นมากเกินไป

- มีการหลั่ง growth factor เช่น TGF-β (transforming growth factor beta) กระตุ้นการสร้าง collagen

- เกิดการแข็งและหนาตัวของเนื้อเยื่อ สูญเสียความยืดหยุ่น

สถานะทางพยาธิวิทยา - Fibrosis เป็น ความผิดปกติ/โรค ไม่ใช่โครงสร้างปกติ

ทั้ง Fascia และ Fibrosis สามารถเกิดการหนาตัวขึ้นได้จากการ "อักเสบ" เช่นกัน แต่เวลาจะนวดรักษา ก่อนจะนวดต้องตรวจและวินิจฉัยอาการให้ละเอียดก่อนว่า ที่หนาตัวขึ้นนั้นเป็น Fibrosis หรือ Fascia และลักษณะอาการอยู่ในระยะไหนเกิดจากสาเหตุอะไร

*** เพราะไม่ใช่ว่า "พังผืด" ที่หนาตัวขึ้น นวดแล้วจะดีขึ้นเสมอไป

ยกตัวอย่างในกรณีของ Fascia ที่ไม่สามารถกลับสู่สภาวะปรกติดได้อีกแล้ว คือ กรณีที่ ไม่สามารถกลับสู่ปกติได้ (irreversible)

- ถ้าเป็น การอักเสบเรื้อรัง/ซ้ำซ้อน fibroblast ถูกกระตุ้นต่อเนื่อง collagen หนาตัวและแข็ง fascia เปลี่ยนเป็น scar-like tissue

ตัวอย่าง: Dupuytren’s contracture / palmar fascia หนาตัวและหดรั้ง ไม่สามารถกลับคืนโครงสร้างปกติ

เพราะฉะนั้น อย่าเห็นว่าเป็นแค่พังผืด นวดสลายแรง ๆ ยังไงก็ได้เดี๋ยวก็สลายไปเอง ถึงแม้หลาย ๆ คนอาจจะบอกว่า "ฉันก็นวดอยู่ ไม่เห็นเป็นอะไรเลย" - แน่นอนคุณอาจบอกว่าไม่เห็นเป็นอะไรเลย เพราะเวลามันเกิดอาการผิดปรกติ คนไข้ส่วนใหญ่เขาก็จะไม่กลับมาหาคุณอีก คุณเลยไม่รู้ไงว่า ไอ้ที่นวดไปมันก่อความเสียหายให้กับคนไข้ยังไง

ข้อมูลที่เหลือ เดี๋ยวจะเอาไปอธิบายในห้อง พท. ในช่วงเวลา Live สด ท่านใดติดงานตามชมย้อนหลังได้เลย เดี๋ยวจะทำเอกสารเอกใน Database ของห้อง พท. ด้วย
