Neuroplasticity is the remodelling of nerve connections and nerve sprouting. And this helps to keep our brain healthy, and prevents the brain from shrinking.

The phrase 'use it or lose it' applies to our brain, as much as our muscles.
If we don't use certain nerve pathways, they die away, just like a footpath through a field disappears if it is never used.

The principles of neuroplasticity are what physiotherapists harness when working with someone who has had a brian injury or stroke, help them regain movement, by making new nerve pathways using the healthy (undamaged) parts of the brain.

Did you know your brain has an amazing ability to adapt and grow, no matter your age?
Keep it sharp by learning something new—whether it's a language, a musical instrument, or even a new route to work.
Or something simple like brushing your teeth using the other hand or swapping your cutlery around.

Learning something new or changing the way we do something familiar encourages the brain to make more neuronal (nerve) connections.

Never giving your brain something new (novel) to do can result in the brain shrinking, as connections that are not used die away.

Challenge your brain, and it will thank you

Do you struggle to balance in the dark and in busy environments?

Do you know which balance system you depend on?
When you have a vestibular condition, it’s common to depend upon vision for balance.

This works well if you’re in a still environment that is well lit
If you are in a busy environment such as lots of people moving around or moving objects such as escalators it becomes much harder to stay balanced and you will probably feel more dizzy

Similarly, if you are in a dark environment, it may be really difficult to stay balanced as there is nothing for your vision to anchor on

When you have a vestibular condition, it’s common to depend upon vision for balance.

Great to read this article published by Vestibular Disorders Association confirming what we see in clinical practice.

People with PPPD ( persistent postural perceptual dizziness) rely strongly on visual cues for balance.

This means in daily life they may struggle in busy visual environments, with patterns and lines on floors, walls or the wider environment.

The research also shows that people with PPPD rely less on somatosensory input from the feet ( also called proprioception).

It also means that rehab needs to be specifically targeted to reduce visual dependency and improve response to somatosensory input.

Vestibular rehabilitation definitely isn’t one size fits all. I have a lot of patients come to see me and say vestibular rehabilitation doesn’t work. But change their mind when it is properly individualised.

If the vestibular rehabilitation programme is a pre printed exercise booklet that is given to everyone, then unless you are very lucky, it won’t work.

Properly prescribed individualised vestibular rehabilitation programmes do work.

This week is Balance Awareness week.
I am working on my balance living on this boat for the week.

Why does living on a boat test my balance?

We use three systems to balance, proprioception ( the information we get from our body- joints, muscles, tendons etc) vision and vestibular.

On a boat the floor is moving so the time. So the information from my vestibular system becomes less accurate. As my foot is on contract with the floor, without moving, which should mean everything is still, but with the floor moving I am having to make constant postural corrections.

Vision- water is constantly moving, so I am living with a constant peripheral visual flow, which can induce dizziness in some people, but also means that I can’t fixate with vision for balance ( think about how a ballerina fixates on a spot for balance when they spin)

Which leaves my vestibular system needing to be the primary system I am using for balance. Great vestibular.

And then of course from time to time I step off the boat and my systems need to recalibrate to being on land.

It also means I am living off grid with very little internet. Which makes for the perfect holiday but also means I am unlikely to be sharing any more posts for balance awareness week.

We are looking for an associate physiotherapist with experience in neurology, older adults and rehabilitation.
We want someone who has a passion for rehabilitation and working holistically. This is an exciting, multifaceted role working in different community environments and ideal for someone who is innovative and enjoys making rehab interesting and fun.

This role is suitable for someone who is driven, enjoys the opportunity to step outside of their comfort zone, and loves a challenge.

This role includes local community rehabilitation and driving up to an hour each way to see clients. Travel time is well remunerated and mileage is paid on top.

Essential:
Physiotherapy degree, graduate diploma or MSc
Member of the Chartered Society of Physiotherapy (CSP)
Health Professions Council registration (HCPC).
Willingness to learn quickly.
Excellent time management skills.
Excellent interpersonal communication skills, including presentation skills.
Computer literacy.
Full Driving License and access to a vehicle for the role.

Yesterday I spent a lovely evening networking with familiar and new faces at Sintons LLP seminar on virtual reality on neurological rehabilitation.

Really interesting to hear Eloise Ross talk about the case managers perspective of Virtual Reality.

Thank you to Sintons LLP for inviting me to speak on using virtual reality in neuro rehab and the role of NeuroFlex VR headset in assessment and treatment of vestibular ocular motor function. ( I was obviously too busy networking to get any photos of me actually there 🙄)

Optimum Results is part of Physiocare Rehab, which focuses on sleep and fatigue management. Using 24 hour monitoring of physiological data, over a period of several days, to assess sleep quality and fatigue and provide an individualised report and recommendations.
Repeat assessment periods demonstrate the effectiveness of interventions.
