At The Bespoke Clinic, we don't treat perimenopause with a template. We start with you. Your symptom pattern, cycle history, energy, sleep, mood, metabolic health and the pressures you are carrying all matter.
This transition affects the whole system, so our assessment reflects that. We combine clinical experience, careful listening and relevant investigations to understand what's happening in your body at this stage of life.
When appropriate, we arrange targeted blood tests to give us a clearer baseline. While hormone levels fluctuate during perimenopause, reviewing markers such as oestradiol, progesterone, testosterone, thyroid function, iron status and metabolic health can be helpful. These results are interpreted in context, alongside your symptoms and cycle pattern. They are not used in isolation, but they provide useful data to guide safe, informed decisions and to monitor change over time.
In more complex cases, particularly where symptoms are layered or unclear, we may recommend advanced functional hormone testing such as the DUTCH Plus test. This assessment measures oestrogen, progesterone and testosterone metabolites, DHEA, and provides a 24-hour picture of cortisol and cortisone patterns. Rather than showing a single hormone level, it helps us understand how your body is producing, converting and detoxing hormones, and how stress physiology may be interacting with your symptoms. This testing is interpreted within a medical framework and integrated with your history and blood markers. Not every woman requires advanced testing, but for some, it provides valuable additional clarity. It can help make sense of longstanding hormone imbalances and explain many chronic symptoms, that women have put up with or been told are ‘normal’ for them!
