🩺 Understanding Risk Stratification Codes in Your Medical Record

You may have recently noticed a Risk Stratification code added to your medical record. We understand this might raise questions, so here’s a quick explanation.

What is Risk Stratification?

Risk stratification is an NHS-approved system used by healthcare teams to understand how much monitoring or support different patients may need.

It helps ensure everyone receives the right level of care at the right time.

These codes are not diagnoses and they do not change your treatment without a discussion with you.

What are RAG statuses?

To help organise care safely, the system uses colour categories:

🔴 Red – you may need closer monitoring or more frequent reviews

🟠 Amber – your condition is stable but still needs routine checks

🟢 Green – your condition is stable and may require less frequent monitoring

This system helps clinicians quickly identify who may benefit from additional support and who can safely continue with standard review intervals.

Why are these codes added?

They help our team:

• understand your current level of clinical need

• recognise when your status may need updating

• ensure continuity of care between clinicians

• prioritise care safely and consistently

Does this affect my care?

No changes are made to your care or treatment plan without your involvement. The code simply helps guide clinical decision-making.

Do I need to do anything?

No action is required from you.

February’s Numbers 💙

We’re proud to share a snapshot of how we supported you last month — and to say thank you for the trust you place in us every day.

⭐ 95% of patients rated our service positively, placing us well above the national NHS GP average. Your kindness and feedback truly mean everything to our team.

⭐ Our Google reviews averaged 4.7 over the past month, helping lift our overall rating to 3.5 stars. Thank you to everyone who took the time to leave feedback - your comments (even if negative/constructive) help us keep improving.

📩 2,591 Accurx requests received; that’s over 120 patient requests every day. We continue to respond as quickly, thoughtfully, and carefully as possible.

📊 Mondays remain our busiest day, averaging 180 requests, while Fridays average 98.

💬 8,347 Accurx messages sent from the surgery; that's around 417 messages a day covering appointments, advice, results, follow-ups, and general support.

📞 4,878 phone calls answered, totalling 123 hours of inbound talk time. We know digital doesn’t work for everyone, and sometimes a voice on the other end really matters.

⏱ 92.6% of calls were answered within 5 minutes — our team is working incredibly hard to speak to you as quickly as possible, even as demand continues to rise.

🩺 6,109 appointments booked across GPs, clinicians, nurses, and our wider healthcare team; all working together to support your wellbeing.

⚠️ 293 missed appointments resulted in almost 82 hours of lost clinical time. As detailed in our previous post, if you can’t attend, please cancel in good time so we can offer the appointment to another patient.

📢 Before we share February’s figures, we need to address something important.

Last month 293 appointments were missed ❌

That’s almost 82 hours of wasted clinical time ⏳

We completely understand that sometimes things happen and you may not be able to attend.

However, if you know you can’t make your appointment, please cancel in good time 🙏

You can cancel by:

📱 Using the NHS App
📝 Submitting an Accurx form
📞 Calling the practice

⚠️ If it’s short notice, please call us directly. You won’t be able to cancel via the app, and we may not see online forms in time. A quick phone call often allows us to offer the slot to another patient, meaning the appointment isn’t wasted.

If we aren’t informed and the appointment is missed, a DNA (Did Not Attend) marker will be added to your record.

📩 Two DNAs within a three-month period will result in a warning letter, and further missed appointments may lead to removal from the practice list - which is not something we want to do.

✅ Cancelling appointments really does make a difference. It helps us see as many patients as possible during what are already challenging times for both the surgery and our patients.

All services will resume as normal the following day.

We are seeing an increasing number of patients attending or contacting the surgery with severe chest pain and/or difficulty breathing.

⚠️ These symptoms are medical emergencies.

If you or someone else is experiencing them, call 999 or attend your local A&E department immediately.

❗ Please do not travel to or contact the surgery for an appointment.

Any delay in not having the appropriate treatment at the hospital can lead to serious consequences.

📞 Call 999 or go to your nearest A&E immediately if you or someone else has:

* Chest pain or difficulty breathing
* Severe bleeding or serious injuries
* Suspected stroke (weakness of one side, slurring of speech or heart attack
* Seizures
* Loss of consciousness or sudden confusion
* Serious head injuries
* Severe allergic reactions

Type 1 diabetes can happen at any age and if left undiagnosed, it can be fatal. The most common symptoms are the 4 Ts: Toilet, Thirsty, Tired, Thinner.

If you think you or your child has any of these symptoms, ask for an urgent GP appointment or call 111.

Nestlé has issued a precautionary recall of specific batches of SMA Infant Formula and SMA Follow-On Formula due to the potential presence of cereulide.

👉 There have been NO confirmed reports of illness, but as a precaution, Woodview Medical Centre are asking parents not to feed any of the affected products (regardless of batch number) to their baby.

🩺 If the formula has been prescribed (regardless of batch number), we are asking that you please take the product back to the pharmacy and ask us for a new prescription.

🔍 How to check affected batches:
Batch codes can be found on the base of the tin or box (powdered formula) or on the outer box and container (ready-to-feed).

💬 If you have an affected batch:
• Do not use the product
• Follow the advice above if prescribed
• If purchased separately, dispose of the product and contact Nestlé for a refund:
