And chill. Great crowd for our latest needling course up in Edinburgh. I always try to judge updating pods on the safe and effective use of LA vs me nerding out on pharmacological minutiae (vs mythbusting ‘what I was taught at college’). I have a VP in for excision soon. I’m torn between full thickness excision with healing by secondary intention, a single lobe or a bilobed local skin flap with primary suturing. It’s a little on the large side for excision but I’m worried about scar location with a flap. What would you do? (All other options tried and failed so keep banana skin out of your G/D mouth!)

Revision Tibialis Anterior repair in one of my long term patients. A very good primary fix from 2024 (see old pics) vs a fall down the stairs and forced ankle plantarflexion. Welcome back 🫣. Refixed with a LARS tendon graft. Almost fully functional at 4/52 post op.

Following an inversion injury I repaired the ATFL. However, there were still some issues pain-wise. Back to surgery 12/12 later. What is this structure and what is the diagnosis? PS. Pain free two weeks post op (other than normal wound discomfort).

While I love a 1st MTP joint replacement, they do fail over time. The first two pics from one that sunk into the meg head after 3 years; the second fractured after 9. Both were switched out for a new one after a joint tidy up. Fusions are still the gold standard. But I’m hoping to keep these two ladies moving and pain free (again).

’s TikTok edit queue just got a whole lot longer after filming dermatome assessments with me last week. It’s important to know the anatomy though. Today I saw a patient (last pic) with an L4 injury post RTA, numbness in the saphenous nerve distribution and a neurologically induced cavus that needs a hallux IPJ arthrodesis. Sorry, Aidan! Excited to see the final product
