💥 AC Joint Pain vs Shoulder Osteoarthritis – What’s the difference? 💥

Shoulder pain isn’t always coming from the same place – and two commonly confused issues are AC joint pathology and shoulder osteoarthritis (OA).

🔍 What is the AC Joint?
The acromioclavicular (AC) joint sits at the very top of your shoulder where your collarbone meets the shoulder blade. It’s a small joint, but it can cause a lot of discomfort when irritated.

⚡ AC Joint Pathology
This can include sprains, degeneration, or inflammation of the AC joint.
Common signs include:
• Pain right on the top of the shoulder
• Pain when reaching across your body (e.g. putting on a seatbelt)
• Discomfort when lying on that side
• Localised tenderness over the joint

🦴 Shoulder Osteoarthritis (OA)
OA affects the main ball-and-socket joint (glenohumeral joint).
It typically presents with:
• Deep, aching joint pain
• Stiffness and reduced range of movement
• Pain more widespread around the shoulder
• A capsular pattern of restriction (external rotation most limited, then abduction, then internal rotation)

⚖️ Key Differences
• Location of pain: AC joint = top of shoulder | OA = deeper joint pain
• Movement pain: AC joint = cross-body movements | OA = general stiffness and restriction
• Range of movement: AC joint = often fairly preserved | OA = globally reduced

Osteoarthritis (OA) of the shoulder is a common condition, especially as we get older or after previous injury. It occurs when the cartilage in the joint gradually wears down, leading to pain, stiffness, and reduced movement.
Common symptoms include: • Deep, aching shoulder pain
• Stiffness, especially in the morning
• Difficulty reaching overhead or behind your back
• Grinding or clicking sensations
How is it diagnosed?
Physiotherapists use a combination of subjective questioning and physical assessment. One key clinical sign of shoulder OA is the capsular pattern of restriction, which typically presents as:
External rotation most limited → Abduction → Internal rotation least limited
This pattern helps guide diagnosis alongside your symptoms and specific movement testing.

A painful arc typically occurs when raising your arm out to the side (abduction), where pain is felt between around 60–120 degrees. This can be a key sign of irritation within the subacromial space – where structures like the rotator cuff tendons and bursa can become compressed.

💡 Common symptoms include:
• Pain when lifting your arm overhead
• Discomfort reaching for items or getting dressed
• Weakness or reduced movement in the shoulder
• Pain lying on the affected side
