Rushcliffe Social Prescribing

Rushcliffe Social Prescribing Rushcliffe Social Prescribing Service works with individuals to help to empower them to make a change

Social Prescribing enables GP practices to refer patients to a Link Worker, exploring solutions around non-medical issues that underpin their health conditions and subsequent presentation at surgeries. Rushcliffe PCN Social Prescribing Service offers the opportunity for patients to work with a Social Prescribing Link Worker. The service looks to enable patients to better manage their own health, empowering those with social, emotional or practical needs to develop wellbeing solutions. Part of this will be working closely with community resources and making connections as these resources begin to emerge post lockdown. Where appropriate, there is also the offer of collaboratively producing a personalised, goals-based plan aimed at supporting and motivating individuals around behaviour change.

This session was great for showing how to make a cheap, easy and nutritious meal from ingredients that most of us already have in our cupboards at home!

Do you want to learn how to make something new, or even just connect with others over a homemade meal? You can check out our upcoming sessions here: https://www.eventbrite.co.uk/cc/cook-connect-4294253

Overview

Looking to make a change but not sure where to start?

Join us at the West Bridgford Social Prescribing Community Hub and have an informal chat with Nick, our Behaviour Change & Lifestyle Support specialist.

Whether it’s improving your health, building better habits, or just feeling a bit more in control day to day — Nick offers a practical, down-to-earth space to help you get started.

What Nick can support with:
Setting realistic, achievable goals
Building healthy habits that actually stick
Finding motivation when things feel stuck
Exploring physical activity and wellbeing options
Making small, sustainable lifestyle changes
Connecting into local services and activities that support your goals

This is not a formal appointment — just a relaxed conversation to help you figure out what might work for you and what your next step could be.

Nick isn’t a clinical specialist, but he can help you cut through the noise, keep things simple, and move forward in a way that feels manageable.
