Lipoedema, historically seen almost exclusively in women, is increasingly recognised in men, though it remains underdiagnosed. This case series from Brazil highlights five male patients with classical lipoedema features, including bilateral, symmetrical lower extremity fat accumulation sparing the feet.

It demonstrates that conservative management yields significant short-term improvements, emphasising the urgent need for increased awareness and research into male lipoedema.

Is weight loss always the full answer, especially for persistent pain?

This latest scoping review published today, sheds light on a crucial, often overlooked issue in patient care: lipoedema frequently remains undiagnosed in individuals with obesity, leading to significant challenges in treatment and patient outcomes.

For patients undergoing metabolic bariatric surgery, this hidden condition presents a paradox. Despite achieving remarkable weight loss and notable decreases in hip and abdomen circumference after surgery, some patients with lipoedema experience only small reductions in the size of their extremities and continue to report persistent and even increased limb pain.

The findings from 49 female patients reveal that:

• 48 out of 49 patients had lipoedema diagnosed after their bariatric surgery, highlighting a critical gap in pre-surgical screening and awareness.

• Even with a mean excess weight loss exceeding 70%, bariatric surgery alone does not appear to improve lipoedema-related symptoms significantly. In fact, the mean pain score in these patients increased after surgery (from 7.30 before to 7.92 after), rather than decreasing.

This underscores a vital point for healthcare professionals and patients alike: recognising the presence of lipoedema in females experiencing extremity pain and disproportionate fat distribution is crucial. Bariatric surgery, while highly effective for obesity, often fails to alleviate the pain associated with lipoedema. This calls for earlier diagnosis and tailored treatment for lipoedema, especially in those considering or having undergone bariatric surgery, to ensure more comprehensive and effective patient care.

"The Deep Oscillation device from Physiopod UK is very useful for all kinds of conditions. I use this device for scar massage and it gets great results. Fibrosis is softened by the gentle pulses which are delivered using an electrostatic circuit.

Whilst away on holiday, I saw a family member who had been bitten by a mosquito causing a reaction in her vein. There was initial swelling (she saw a medical professional) and then the swelling had reduced, but the vein was stiff and fibrotic. A short treatment softened the tissue and it was already better at the end of the treatment, 24 hours later we re-assessed and it was still improving. Ideally she would have 3 more treatments to get continued improvements."
