Supporting Care Home Teams Through End-of-Life Care

I would like to share a scenario which unfortunately seems to be happening more often than it should.

I reviewed one of our lovely residents Premium Care Group who had been discharged from hospital. It’s Saturday and they have been slowly deteriorating, and now stopped eating and drinking and taking their medications. The care staff felt they were nearing the end of life, but no treatment plan in place. No recent GP review and 111 asking care staff to contact their own GP after the weekend. This left the staff and family worried and stressed, feeling unprepared to manage his needs and how to progress.

Can you relate to this at your care home?

I was able to review the resident over the weekend, and leave staff with clear guidance on symptom management and treatment plan. I was able to prescribe and provide and required medication. The care staff felt immediate relief, knowing they’re providing him with the best care. Our support allows them to focus on his comfort rather than stressing over medical decisions, bringing peace to the resident, their family and the care staff.

Meet the lovely Iris. She was so happy we were able to resolve her new medical issue quickly and efficiently. The staff at Premium Care Group were happy as they did not have to take up some of their valuable time trying to get help for Iris. The comfort of knowing a doctor is coming on the weekend cannot be overstated.

We work as a team to look after our residents and they couldn’t be happier.
