I don’t know about you guys, but 50% of my day is spent on FUNGUS. 

Fungal nails, fungal skin, things that look like fungal nails and fungal skin (that aren’t)

I was compiling a list of questions and things I wanted to look into in more depth… then I thought I could cut a massive corner by just asking Dr. Ivan Bristow about it.

And because I’m a sharing, caring kinda gal I thought it’d be a good idea to turn it into a free webinar so that any interested peers could come along and ask any burning questions of their own - and Ivan agreed. Hurrah!

Sepsis!

According to the CDC, "Sepsis is the body's extreme response to an infection. It is a life-threatening medical emergency. Sepsis happens when an infection you already have triggers a chain reaction throughout your body. Infections that lead to sepsis most often start in the lung, urinary tract, skin, or gastrointestinal tract".

Signs
Shakes and shivers
Generally feeling unwell
Feeling dizzy
Person is talking nonsense and/or or is acting out of character (the classic response of, "NO, I'M FINE", when they are, quite clearly, not fine)
Feeling sick
Muscle pain
Breathing may be affected

Facts
It is not infectious
It is life-threatening
Anyone with an infection can get sepsis (septicaemia/blood poisoning)
Immunocompromised-folks, including people with Type 2 diabetes, are more at risk of sepsis

Treatment
It is important to identify sepsis quickly and get the person to hospital where they will be urgently treated with antibiotics
If there is a delay in the adminstration of antibiotics, a person can go into septic shock - and their organs can fail. This could be really bad news ☠️

Recovery
If identified and treated quickly, most people will make a full recovery.

Retronychia! Yowzers.

A different type of ingrowing toenail - the nail plate ingrows into the proximal nail fold (the thickened skin at the base of the nail). It is thought to be second to persistent infections (paronychia) and/or trauma (e.g. walking downhill for a prolonged period in tight boots).

Treatment
Acute: Hydrocortisone 1% cream applied to the nail base to calm things down
Address any active soft tissue infection (antibiotics)
The nail may naturally avulse (fall off); and/or a new nail may start growing beneath the injured nail (which will eventually push the 'dead' nail off).

Chronic: If the problem recurs, or fails to settle with conservative treatment, the nail can be surgically removed.

May is Skin Cancer Awareness Month 🧐

Check those lumps n bumps, and if you're not sure, make an appointment with your GP or specialist podiatrist whom will be able to advise you further and refer you if necessary.

Any new or changing moles or blemishes should be checked out - particularly if they are: -

Asymmetric - ill defined borders and different colours and/or topography (bumpiness!)
Borders - these may be changing over time
Colour - different colours rather than a uniform brown, for example. A blue-y grey tinge should be checked out ASAP - speak to your GP
Diameter - is it getting bigger?
Evolving - the lesion is changing quite quickly

Many podiatrists are trained in Dermoscopy and will discuss with you at your appointment if you have any lesions of concern, and refer you for further investigation.

In Oxfordshire, you will be seen at Dermatology by a specialist within 2 weeks for any suspicious lesions.

The quicker a dodgy lump is recognised, the better the outcome - so don't just ignore it!

About Felicity

I qualified from the University of Southampton with a BSc. (hons) Podiatry in 2010.

Since graduation I have worked both within the NHS and private sector. Within the NHS, I specialised as a Community Diabetes Lead Podiatrist working in community clinics, the Churchill Hospital's diabetic foot clinic and domiciliary settings. I have previously had permanent roles within Oxford Health NHS Foundation Trust and have locumed in Greenwich, Ealing, Kensington, Milton Keynes and Buckinghamshire.

I waved goodbye to the NHS in June 2017 and now offer private care within your home across Oxfordshire and West London. I also work at The Summertown Clinic on alternate Wednesday evenings and ad-hoc Saturday mornings if you would prefer to be seen in clinic.​

I have undertaken further study in rheumatology, casting techniques, biomechanics, x-ray and blood test interpretation, acupuncture, manicure and pedicure and hold a further undergraduate degree in physiology (University of Southampton, 2007). I am currently looking to undertake a masters degree in Human Nutrition.​
